The Brook Supportive Housing Program has opportunities Social Workers to join an established clinical team. The Brook permanently houses 189 tenants; the majority are formerly street homeless and have serious mental and medical illness (including HIV/AIDS) and substance abuse challenges. Social Workers provide psychosocial support, medication monitoring, financial management assistance and supportive case management services to single adults in a community environment. This Senior Social Work Vacancy has supervisory responsibilities and will carry a case-load.


• Supervise BA/BS level case workers

• Provide on call coverage for team’s case load

• Particiapte in the training and recruitment of new staff

• Provide timely services to the resident that are consistent with those recommended and described in the HRA-approved application (as applicable), service plan and reviews.

• Develop service plan reviews with the consumer as well as review documentation and assessments produced by BA/BS Level Case Workers

• Coordinate services with the resident's treatment and other service providers and make referrals and linkages to additional services for the consumer when necessary.

• Actively engage the resident who is absent or stops participating in services by conducting appropriate outreach based on the resident's needs, to maintain continuity of services.

• Provide case oversight and task supervision


Ideal candidate can demonstrate ability to work in partnership with clients from a strengths-based perspective, overcoming stigma, and navigating complex systems. Additionally, candidate can adapt to an active environment, is exceptional with time management, has strong writing and communication skills, exhibits excellent attention to detail and remains a team player. Opportunities for group work are also available.


The Brook provides a high level of support to its Social Workers through weekly individual supervision, rounds, on-site trainings and staff meetings. The Brook supports continuing education and career growth through individual and group LCSW supervision, as well as opportunities for earning Continuing  


RESPONSIBILITIES

• Supervisory responsibilities and manage assignment of clinically complex clients and provide psychotherapy, crisis management and supportive counseling.

• Provide additional supplementary services such as running groups and assisting with staff training.

• Provide psychosocial support to a diverse client population.

• Perform medication monitoring to clients in need of support.

• Conduct financial management assistance to program participants.

• Provide supportive case management services to single adults in a community environment.

• Work some evening hours and weekends by assuming leadership responsibilities as extension of Clinical Coordinators during Saturday and evening shifts.

• Remain available for client assistance as needed or until interventions are completed.

• Perform other duties as assigned by manager.

QUALIFICATIONS

• A master’s degree in social work and LMSW certification is required.

• MSW candidates are welcome to apply, but must obtain license within 3-6 months of hire.

• A minimum of 1-year of experience working with individuals who have experienced street homelessness, severe and persistent mental illness or substance abuse.
