A Place Called Home has a great opportunity for a passionate and dedicated Case Manager to come and implement creative ways to deliver case management that will shape the future of the youth participating in our programs.



A Place Called Home is a dynamic, non-profit youth and community center in South Central Los Angeles. APCH provides educational programs, counseling, mentoring, music, dance and art classes, fitness and health and wellness opportunities. As youth participate in our programs, they discover interests and talents and develop a better sense of themselves and their place in the world around them. Our goal is to increase the likelihood that youth will stay in school, pursue higher education, attain viable jobs, and become active community members capable of making positive change in the world in which they live.

Our mission is to serve youth and families directly and the optimal way to do that is in person. The majority of our positions require employees to work onsite at our facilities. We will always comply with or exceed government and public health safety mandates. Our priorities and policies to protect the safety and health of our staff and members may require special protocols on our campus and/or may require us to work remotely on a temporary basis. 


APCH COVID-19 Vaccination Policy

A Place Called Home will be mandating COVID-19 vaccines for our staff. Consistent with our mission and values, and the duty to provide and maintain a workplace free of recognized hazards, A Place Called Home (“APCH”) has adopted this policy to safeguard the health and well-being of our staff and our families, members and their families, volunteers, vendors, and others who spend time on our campus, and the community, from the risks associated with COVID-19. All new hires must provide proof of their first dose of the vaccine on the first day of employment, if not fully vaccinated, and will have 30 days from their date of hire to complete their COVID-19 vaccination and submit proof of completion to the organization’s HR department.

Department: Teen & Young Adult Services 

Reports to: IMPACT Program Manager                 

FLSA Status/Hours: Non-Exempt, 40 hours/week (Must work Saturdays)


Under the direction of the IMPACT Program Manager, the IMPACT Case Manager provides oversight, resource support and behavioral management support for our IMPACT members and families. The IMPACT Case Manager monitors the member’s experience, advocating on behalf of the member’s human services needs. The IMPACT team works very closely with our RISE School Partnership that is a continuation school that is housed on APCH’s site; students of which are housing unstable, homeless, justice involved or have difficulties functioning at a traditional school. As a part of the IMPACT team the IMPACT Case Manager will be responsible to assist with addressing escalating situations to respond and care for members. 

The IMPACT Case Manager is responsible for leadership in developing individual program plans in response to member’s behavioral needs. The IMPACT Case Manager will identify, prevent or de-escalate injurious and destructive behavior in the classroom and on campus and will provide behavior intervention and socio-emotional support in the most severe behavior circumstances. {IMPACT program specifically targets individuals from the following populations: Homeless, Housing unstable, Credit deficient, Justice Involved, Foster and Group home individuals from the ages 14 - 22. } The IMPACT Case Manager is responsible for creating behavior modification plans and tools for various members across the campus site, particularly focusing on the Opportunity Youth population (RISE High and IMPACT programming). The IMPACT Case Manager will guide the members in creating and completing SMART goals, identifying both short and long term goals. The IMPACT Case Manager assists in the design and implementation of behavior support plans; and works with the Counseling team and all staff to implement and monitor the progress of intervention plans in accordance with A Place Called Home’s policies and REACH values. This position will coordinate with the RISE partners and the Counseling team to support, advocate and connect APCH’s youth and their family members to resources according to need.


Case Management

  • Under the direction of the Program Manager and in collaboration with Membership and Counseling departments, facilitates the member intake process, which includes a needs assessment, SMART goals and complete a Member Intervention Plan (MIP) that outlines the program plan that aligns with their goals. 
  • Conduct 1:1 meetings on a monthly basis with members to evaluate progress towards goals, identify challenges and solutions, and to adjust the program plan as needed. 
  • In collaboration with Ed Services, Higher Education, and Professional Readiness and Employment staff, monitor progress members are making on educational and/or transitional plans, MIPs and goal setting sheets and update Program Manager on a consistent and as needed basis
  • Manages case load (case management) between 25-50 members 
  • Responsible for the accurate and timely record keeping
  • Must maintain an accurate case management tracking database in which each profile includes an Inquiry Form, SMART Goal Setting Sheet, Needs Assessment, and Member Intervention Plan and other documents as required; update case files within 48 hours of meeting with members
  • Responsible for administering the youth assessment tools (e.g. Adverse Childhood Experience (ACE) Questionnaire or the Beck Depression Inventory), and other onboarding documents as required
  • Works closely with Program Coordinator to provide member referrals and support with the dissemination of resources when necessary 
  • Maintain confidentiality requirements at all times
  • Provide referrals to resources and service both internally and externally
  • Performs related duties as assigned

 Coordinate Community Resources

  • Assists with researching and connecting with potential partner organizations for purposes of resources, special opportunities, and to build and maintain a referral base for direct connect.
  • Research and be involved in connecting IMPACT youth to community resource events  
  • Support holistic care for members by connecting each member with appropriate APCH departments and resources
  • Assist with identifying needs of the program and members
  • Responsible for providing referrals to resources both internally and externally
  • Support expense tracking for all resources purchased by IMPACT program to ensure accurate tracking of grant expenditures
  • Coordinate and disburse internal resources to members 
  • Collaborate with the In-Kind team and Counseling department to maintain an accurate inventory of resources available to members on-campus

Program Coordination 

  • Collaborate with the Program Manager and Program Coordinator to set the calendar of events, special opportunities and program offerings for each semester
  • Assist Membership with placing member in the appropriate classes each semester as outline in individuals’ Member Intervention Plan
  • Ensure Member Intervention Plans are disseminated to support team and instructors 
  • Work closely with Program Manager to develop and disseminate information regarding IMPACT program offerings and special opportunities on a monthly basis or as required
  • Communicate with members & members’ stakeholders (Probation officers, foster parent/guardian, instructors, etc) about upcoming events, forms that need to be submitted, transportation needs, etc. 
  • Assist contractors and staff in classroom management
  • Support Program Manager in data collection/analysis of program and member evaluations as required

Member Care

  • Monitor and ensure members remain in designated locations on agency campus
  • Circulate throughout campus including instructional spaces and general areas to serve as a consistent and supportive presence to members throughout program hours
  • Provide mediation in conflict situations between members to prevent behavioral crisis.(Connecting with members who are escalated and providing therapeutic outlet and processing of feelings.)
  • Monitor members to detect unusual or harmful behavior and report potential escalation of behavior to the security and ensure a detailed incident report is submitted to the Counseling Team, Program Director and Senior Program Director prior to the end of the program day. 
  • Oversight of support staff and volunteers for special events and serve as primary chaperone as necessary
  • Prevent and de-escalate harmful behavior in a behavior crisis situation to reduce members’ risk of injury and ensure safety of others, using Nonviolent Crisis Prevention and Intervention (CPI) strategies 
  • In most severe circumstances, where member behavior presents immediate safety concerns, provide physical restraint (learned via appropriate training) as means of removing member from the situation and de-escalation
  • Model communication and interaction that respect and include all individuals and their languages, abilities, religions, and cultures
  • Identify when members need more clinical support and refer to APCH Counseling Department 
  • Responsible for building rapport, inspiring and finding unique ways to offer hope for members
  • Documents members progress and shares progress with IMPACT team during - case review conference. 

Administrative Support

  • Maintain database of members attendance and program participation notes 
  • Being knowledgeable of the current budget when suggesting or requesting certain services or product
  • Support with gathering relevant data and information for program reports and updates
  • Submit member attendance & submit daily program report 
  • Create and submit paperwork and monthly reports in a timely fashion
  • Communicate effectively, both orally and in writing; 
  • Attend all required meetings (Staff, Supervisor, team, etc.)
  • Support other team members in carrying out center protocols, rules, and policies
  • Administrative duties as assigned

Other Non-Essential Responsibilities

  • Assist with the supervision of members during program hours, special events and field trips
  • Serve as a mentor and role model for members
  • Attend all required program meetings and events
  • Perform other tasks as assigned

Education/Training/Experience (Edit according to where position falls in the Career Graph)

  • Three years of experience in providing behavioral intervention or behavior management services to moderate to severe emotionally-disturbed individuals.
  • (Bachelor’s degree preferred)Associate's degree or equivalent of 60 semester or 90 quarter units in psychology, mental health, behavior management, special education, or a field closely related to the knowledge and abilities of this classification, from an institution of higher learning recognized by the Council for Higher Education Accreditation. 
  • Certification in Crisis Prevention and Intervention (CPI) is also required and needs to be completed within 90 days of hire if not already certified
  • Equivalency Provision: Certificate in a psychiatric technician or other similar mental health program from an institute of higher learning recognized by the Council for Higher Education Accreditation may substitute for one year of the required experience.
