  • Participate in Individual and Group Sessions with peers under the Leadership of the Partnership/Relationship Development
  • Build strong relationships and collaborations with participants, families, CBO staff, School Leadership and external institutions (school, social service, mental health providers, etc.)
  • Cultivate and make internal and external referrals for services as needed and provide regular follow up
  • Meet with Principal/School Leadership, Director and Assistant Director for case review once a month
  • Serve as a Mandated reporter and follow the chain of command for reporting suspected cases of abuse or neglect


Program Development

  • Maintain a caseload of 25-35 participants providing individual and group crisis intervention and case management services 
  • Develop focused, achievable individual service plans that outline short- and long-term goals
  • Conduct needs assessment of youth and prioritize objectives
  • Provide one-on-one coaching sessions that guide youth through stages of transformation and socio-emotional development.
  • Identify and provide internal/external referrals to families as needed 
  • Coordinate and facilitate groups designed to address youth challenges and lead to self-understanding and change
  • Support Group Leaders with behavior management techniques and strategies to assist students in a group setting 
  • Have the ability to adhere to a changing environment as needed and continue to provide services in person and virtually through Google Classroom or other virtual learning platforms


Administration

  • Maintain case records, schedules, track inputs/student progress and data integrity (data accuracy, timeliness of entry into the data system, and completeness) into ETO and Youthservices databases
  • Use SEL assessments to make informed decisions with the goal of continually improving service to youth and progress toward intended outcomes 
  • Ensure compliance with the requirements of various funding sources
  • Conduct quarterly SEL assessments by the required benchmarks
  • Attend all Division wide Leadership Meetings
  • Licensed Clinical Social Worker
  • Attend 21st CCLC Quarterly Advisory Board Meetings
  • Maintain a 21st CCLC referral log
