Position Title: Rapid Re-housing- Case Manager
Reports to: Assistant Director and Project Coordinator
Location: Housing First Division Office
Developed by: Division Director
Classification: Exempt- Administrative
Hours: Monday – Friday 9:00 AM – 5:00PM, some evenings, weekends and holidays may be required.
Participation in an emergency on-call rotation.
This position provides Rapid Re-Housing services to single individuals experiencing
homelessness in DC through the Rapid Re-Housing Grant through the DC
Department of Human Services. The Rapid Re-Housing program follows a Housing
First model and philosophy. The Case Manager will utilize best practices and
consumer centeredness at all times. The Case Manager will assist Individuals in
obtaining permanent housing. Case managers will provide services to Individuals
over a 4 month period. The Case Manager will work to empower individuals
experiencing homelessness in DC to become stably housed.
Bachelor’s degree in social work, psychology, counseling, or related
service/science or health care discipline and 1 year of experience in case management
High school diploma or equivalency with 6 years of experience in social services and/or human services.
Certification and/or licensure in a relevant discipline may also substitute for educational requirement.
Understanding and/or experience working with the homeless.
Understanding of adult rehabilitation values and principles.
Understanding of mental health and drug addiction.
Experience with HMIS data entry preferred.
Strong written and verbal skills.
Ability to work in a culturally diverse environment.
Ability to work well under pressure.
Ability to work as a team player.
Valid driver’s license and vehicle are required.
Provide rapid re-housing services to individuals experiencing homelessness.
Develop, implement and revise 4-month housing stability plan with participants.
Provide case management as it pertains to housing stability plan.
Deliver face-to-face contacts to participants.
Foster wellness and independence.
Engage participants and assess individual needs, preferences and strengths.
Assist participants in locating permanent housing.
Make appropriate referrals to identified needed services.
Act as a liaison with landlords and other community stakeholders.
Collaborate with external agencies providing services to consumers, including community and government
Participate in multidisciplinary meetings and regular supervision.
Maintain paper files
Maintain consumer electronic records in the Homeless Management Information System (HMIS).
Demonstrate empathy and flexibility in responding to consumers’ needs and requests.
Attend professional trainings and conferences as needed.
Perform all other duties as assigned.
Employee must have his/her own transportation to carry out job duties and responsibilities. The vehicle must meet legal requirements (insured, registered, etc.) and the employee must have a valid driver's license.
