The Case Manager conducts assessments, determines eligibility for various programs, develops individualized case plans, links individuals and families with community-based housing and service resources, assists in budgeting and manages access to financial assistance funds (security deposits, rent), and provides comprehensive case management services in conjunction with community partners.
· Meets with clients and collects all documentation required for a thorough intake of each client.
· Researches any options for temporary housing opportunities for eligible participants and where possible, explores any permanent housing options for clients.
· Provides case management services including, but not limited to: providing appropriate resources, tools, and counseling to assist participants in achieving their case plan goals; making referrals for services; following up to ensure that the referrals were completed and tracking and documenting participant progress.
· Collaborates with other agencies and support resources to maximize participant outcomes and program goals.
Bachelor’s Degree in social work, human services, or a related field preferred, but consideration will be given to candidates with extensive experience. Case Management and/or client intake experience preferred. Proficient with Microsoft applications; Word, Excel and data base systems. Precise record keeping, excellent oral and written communication skills and a commitment to teamwork is essential. Must have a valid MA driver's license and provide own transportation. Some community outreach will be required.
