The Bridge, an innovative and well respected behavioral health agency serving adults with serious mental illness, substance abuse, co-occurring mental health and substance abuse disorders, and co-morbid medical conditions is seeking a Clinical Staff Worker for its Outpatient Behavioral Health Program.

Responsibilities:

  • Responsible for the chart management of PROS participants. Engage participants in a trusting, collaborative relationship directed toward the attainment of a meaningful life role goal; Provide individual and group clinical counseling, skill-building, support, and therapy services, including admissions, assessments, treatment planning and discharges utilizing evidence-based/best and trauma informed practices
  • Develop and implement group and individual services, including identifying and/or creating curricula that meets the needs and interests of service recipients; collaborate with the client in the creation of individualized service plans, and crisis/relapse prevention plans; help participants link their strengths and preferences to achievement of life goals
  • Collaborate with a multi-disciplinary team to provide participants with person-centered recovery and mental health services; document all participant contacts and services (Individual Recovery Planning, progress notes, employment info etc.) in electronic health record in a timely manner and reports necessary outcomes to governing bodies (CPI, OMH, etc.)
  • Provide services to assist clients with needs such as family support, housing, entitlements, employment, education, vocational interests, community integration, and connection to physical health care; pererform other related duties as assigned by Program Director or Administrative Leadership

Education/Qualifications:

  • Master’s degree in a social service-related field from an accredited college and one year of professional experience ; NY State licensure (LCSW/LMSW, LMHC, LCAT, etc.) strongly preferred
  • Strong professional boundaries are a must; visibility of work requires the ability to work in a fast-paced environment, multi-task, and attention to detail; must have excellent verbal and written communication skills, professional presentation, and excellent interpersonal/customer service skills
  • Must have knowledge of the current software including Word, Excel, etc.; knowledge of AWARDS, PSYCKES, and Healthix preferred
  • Strong presentation and networking skills; must be a team player and have a strong work ethic; must be flexible, creative, and energetic; ability to navigate difficult situations
