Job Summary: The CARE Case Manager is responsible for supporting wellness and the psychosocial emotional needs of students and families in our program.
Key Responsibilities:
● Facilitate communication between the agencies, students, parents/families
● Provide strengths-based case management for each client
● Manage and maintain client files and archives
● Maintain an uncompromising commitment to 100% client confidentiality
● Provide one-on-one support to individual students, parents/families, and alumni
● Be able to provide services to clients virtually as needed
● Build effective and empowering partnerships with the community
● Maintain updated case documentation and student database with attention to detail
● Prepare monthly reports as determined by the Director of CARE
● Provide socioemotional support individually and in group settings
● Conduct assessments as needed
● Provide social service referrals to families and follow up to ensure services were provided
● Provide crisis intervention services
● Facilitate trainings, workshops, and meetings to parents/caregivers in the program
● Facilitate professional development training for staff and teaching artists
● Attend parent and community meetings at various sites
● Attend periodic training and professional development opportunities
● Facilitate a support group for youth and/or parents as needed
● Keep current on applicable child development practices to better assist families
● Attend weekly staff meetings and bi-weekly meetings with supervisor and CARE team
● Maintain ongoing working relationships with all program staff
● Flexible work schedule to include some weekends/weeknights
● Contribute to the development of the CARE Department Curriculum
● Assist the Director of CARE and team with developing new innovative social justice and anti-racist oriented practices and relationships
● Other tasks as assigned based on the needs of the organization
● And other duties assigned by the Director of CARE
● Master’s degree with a minimum of three years of relevant professional experience in social work, family counseling, psychology, case management, child development, and/or a license in any of these fields
● Ability to exercise good judgment and act calmly in emergency situations
● Has excellent verbal, written, and phone skills in both English and Spanish
● Has an empathetic approach towards families and a high level of personal integrity and commitment to excellence
● Ability to represent Harmony Project values such as commitment, community, collaboration, respect, responsibility, integrity, and sustainability
● Fluency in all Microsoft Office programs, as well as Google Workplace (formerly G Suite)
● Work independently and collaboratively with staff, adhering to important deadlines
● A current driver’s license and reliable means of transportation to do site visits as needed
● Ability to communicate effectively and with cultural sensitivity to a diverse community
● A desire and ability to be self-driven and take initiative on projects
