Lifelong is a non-profit community health organization committed to providing practical support services and advocating for those whose lives are affected by chronic health conditions. The East King County System Navigation Hub is a regional program funded by the Veterans Seniors and Human Services Levy (VSHSL) that provides person-centered services and resources to low-income adults with disabilities and their caregivers through a network of community partner and service providers to support stabilization in the community and increase quality of life.


Job Description

The Project Manager is a role to launch of Lifelong’s System Navigation Hub in East King County.

The role is responsible for hub partner recruitment and relationship-building, community marketing, system development and implementation. The work requires the ability to communicate clearly both orally and in writing, maintain positive and productive relationships with community partners, maintain professional boundaries, and effective time management with varying day-to-day activities and related workflow. The position reports to the Director of Aging and Disability Services. This is an exempt, non-union, regular and full-time position.


CORE RESPONSIBILITIES:


Needs-Based Assessment

In partnership with Quality Assurance department and Director of Aging and Disability Services, develop community-based needs-based assessment strategy with goal of collecting feedback on needed services and resources from adults with disabilities and their caregivers as well as community-based service providers and stakeholders throughout East King County. Facilitate needs-based assessment and develop analysis and reporting on results to support recruitment of hub partners and focus practice areas.


Networking

Recruit hub partners and achieve memorandums of understanding with direct service providers to participate in hub structure. Develop and maintain positive and productive working relationships with hub provider partners.

Educate potential partners and community allies on purpose and mission of System Navigation Hub. This may include establishing collaborative relationships with community-based social service providers, community centers, health care providers, advocacy organizations, and other stakeholders as universities and government entities. Recruit consumer advisory board members. Consult with Director of Aging and Disability Services as needed.


Administrative and Data Reporting

Develop shared care plan template and quarterly service linkage report design to be distributed to hub partners on quarterly basis. Develop and oversee data tracking and reporting system according to internal agency and external funder requirements. Consult on and conduct database projects as assigned. Develop and oversee consumer advisory board expectations and structure.


Communication

Proactively communicate with all program staff, interagency staff, and community partners and providers as needed. Communicate with hub service recipients and their caregivers including other formal or informal supports to perform duties of the position. Identify necessary communication methods and strategies for overall hub service functions. Provider regular reports to program manager and agency leadership. Attend and participate in meets with King County staff and other VSHSL funded partners as needed.  


May assist with other project work and duties as assigned.


QUALIFICATIONS:


  • Bachelor’s
  • degree required; degree in a human or social services, public administration,
  • or related field preferred.

  • Direct
  • human or social service experience required; experience with King County
  • social services and related system navigation preferred.

  • At least
  • two years experience in human or social services; program coordination or
  • project management experience with specific experience in community network
  • and partnership-building preferred.


·      Able to organize, prioritize multiple projects, and meet deadlines in a time-sensitive environment.

·      Demonstrated skills and strong working knowledge of Microsoft Office suite of applications, including Excel, Access, and Word.

·      Ability to communicate clearly and professionally in both verbal and written form.

·      Proven ability to work independently and in a collaborative team environment.

·      Ability to develop good working relationships with internal and external customers.

·      Ability to understand and model the mission and ethics of Lifelong and the Aging and Disabilities department.

·      Ability to lift up to 25 lbs.


All qualified candidates must possess a valid drivers’ license and have access to a vehicle each work day. Mileage will be paid at the current Internal Revenue Service rate when staff is assigned to conduct business in their own vehicle.
