OUTREACH CASE WORKER

The Outreach Case Worker advocates for a caseload of clients receiving care for mental health issues. Provide outreach to clients wherever they live, (may live with family, by themselves, in shelters, in sober houses, in a hospital, short term rehab, etc).

Develop, implement & monitor quality of all treatment ensuring the physical and mental well being of clients. May require fiscal management of individual’s funds; documentation of treatment plans, ordering and distribution of medication, management/oversight/upkeep of individual’s clinical/health records at the residential site. Transport clients. Serve as primary liaison for service providers, family and guardians.

REQUIRES: current valid drivers license in good standing with driving experience & solid driving record. Must have access to your own personal vehicle for outreach and client transportation as needed.

EDUCATION/QUALIFICATIONS:

• HS Diploma, Bachelor’s degree in Psychology, Human Services or rel field preferred
• Minimum of 1 years experience in the Mental Health field. 
• Working knowledge of community based treatment, funding sources, and treatment regulations, desirable.
• CPR & MAP certifications will be required; we will provide training.
