Program Overview:

Based on the success of our Refugee Health Collaborative model, NSC has expanded our health access work for immigrants, asylum seekers and others. This program offers needs assessments, referrals, and advocacy for clients in need of primary, specialty, and mental health care. The team’s Health Access services to any NSC clients include connecting clients with primary care and specialist providers, coordination of care for clients accessing dental care through NSC’s collaboration with Penn Dental Medicine or other dental providers, and addressing other healthcare-related needs, such as insurance eligibility screening/enrollment and medical bills.

About the Position:

The Health Access Coordinator will be responsible for the work of the Health Access team, which supports NSC clients of any NSC program with their healthcare needs. In coordination with the Clinic Liaison and the Manager of INSPIRE Services and Refugee Health, the Health Access Coordinator will also support some aspects of the specific services to refugees in their resettlement period. The Health Access Coordinator is responsible for the day-to-day operations of the Health Access programming, including operations, staff supervision and support and quality assurance. The Health Access Coordinator supervises year-long volunteers and interns on the Health Access team to address client health-related needs. This position also provides training for incoming year-long volunteers on the Health Access team.

The Health Access Coordinator position has internal contacts with the entire administrative staff and external contact with clients, clinic staff, representatives of other social services agencies and the community. This position has access to sensitive NSC and client information and is expected to handle such information with integrity and professionalism. This position is expected to represent NSC in a professional manner.

The Health Access Coordinator will report directly to the Senior Manager of Health Access and Specialized Supports and will participate in department activities, as necessary.

Primary Responsibilities:

· Provide leadership of Health Access team and supervise team members and student interns.

· Manage and assign Health Access referrals to team members and coordinate with referring case managers as appropriate.

· Manage the Health Access team weekly schedule of client appointments and manage assignment of appointment accompaniment.

· Connect new refugee arrivals and other clients with WIC (the Supplemental Nutrition Program for Women, Infants and Children), as appropriate.

· Provide backup support for Clinic Liaison for coordination with refugee clinics as needed.

· Provide backup support for Health Corps members’ duties during Health Corps members’ yearly transition and monthly member meetings.

· Provide documentation of all activities through case file forms and electronic record management entry. Develop a quality assurance plan and monitor team documentation in Apricot (client record management system).

· Provide onboarding training and ongoing assistance to Health Access team members.

· Troubleshoot healthcare access questions with team members to assist clients, including transportation issues.

· Manage Health Access program budget and reporting to funders.

· Provide NSC staff outside of Health Access team with information and resources to support more involved healthcare needs, such as medical bills and Emergency Medical Assistance applications.

Qualifications:

NSC’s staff is comprised of and seeks individuals with diverse professional and lived experiences, fluency in multiple languages, and a deep knowledge of and commitment to the immigrant communities we serve.

Applicant should possess the following: strong communication and interpersonal skills as demonstrated by compassionate, cooperative, and professional interaction with diverse groups of co-workers, external business partners, and the community; strong ability to adapt to the use of computer and use a variety of common software programs. Knowledge of basic client management procedures for determining eligibility, assessing needs, identifying resources, making referrals, and documenting client interactions and proactively supporting client’s pursuit of goals. Working knowledge of the US health care system, 1+ years working with immigrants/refugees preferred. BA/BS degree preferred by not required from an accredited college or university in a related field of study. MPH or MSW preferred but not required. Overseas credentials or other commensurate experience accepted. Valid drivers’ license with access to reliable transportation or eligible for shared auto program.
