 Sunnyside Community Services (SCS), an innovative, community based, social service organization in Queens (a 10 minute subway ride from Grand Central) is seeking a Friendly Visiting Coordinator.

Program Description: The Friendly Visiting coordinator works with Citymeals and the Department for the Aging in identifying volunteers who are interested in visiting homebound elderly who are at risk due to their social isolation. The coordinator receives referrals from the Case Management program who have been identified as at risk.  

Role Description: A key part of the position is the coordination of programs that encourage local volunteers to connect with their homebound elderly neighbors on a consistent basis through friendly visiting and its variations.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Recruit volunteers to visit homebound elderly.
  2. Working with the Sunnyside Administrative Program Assistant, administer the volunteer screening process including interviews and reference checks.
  3. Train volunteers on the needs of the homebound elderly and emergency protocols.
  4. Maintain ongoing communication with friendly visiting volunteers via phone calls and emails and with the homebound elderly with in home visits and phone calls.
  5. Manage the matchmaking and introduction process for the volunteer and the senior.
  6. Evaluate the success of the match.
  7. Recognize local friendly visitors with emails; thank you notes and events.
  8. Work with Citymeals and Department for the Aging to receive referrals from their volunteer departments.
  9. Report to Citymeals and Department for the Aging monthly and quarterly, the total amount of matches and friendly visiting hours.
  10. Attend Citymeals and Department for the Aging training sessions.
  11. Provide leadership and oversight of the Pet Pals program
  12. Other tasks as needed.


Qualifications:

 BA in Social Work or related field of human services and at least 3 years’ experience in the field of human services with 2 years on an administrative level, preferably in the field of aging.

Microsoft office applications, especially Outlook for email, calendar, contacts. Other applications helpful including Excel, internet, research tool

Familiarity with electronic medical records.

PHYSICAL REQUIREMENTS:

Local travel via public transportation is required on occasion.
