Located in East Harlem, the Center for Comprehensive Health Practice (CCHP) integrates high quality primary care, substance use treatment, behavioral health and supportive services all under one roof. We are seeking a Social Worker for the Pregnant Addicts and Addicted Mothers Program, PAAM, to help individuals struggling with substance use and co-occurring disorders through caring, integrated and evidence-based approaches. PAAM was among the first in the nation to offer truly comprehensive care to heroin-addicted women and their families. The overall goals are for each woman to curtail substance abuse; receive physical and emotional health care; improve economic self-sufficiency; be an able parent; and, if pregnant, deliver a healthy baby. An individual service plan is developed to meet the specific needs of each woman. Women are encouraged to enroll with the knowledge that services are designed specifically for women.

The Social Worker will provide individual and group therapy, crisis intervention, case management, client advocacy, and harm reduction. The goals of the program are to help patients maintain abstinence from illicit drug use while addressing their medical, mental health, family, interpersonal, housing, legal and vocational needs.

Level of Language Proficiency

At CCHP, we value and practice a team-based approach to care. Experience working in a team is not required, but is key to the Center's mission, in addition to:

  • LCSW, LMSW (clinical supervision will be provided by an LCSW), LMHC and/or CASAC
  • Experience working with underserved and substance use populations preferred, but not required
  • Bi-lingual Spanish/English required
