Pathways to Housing DC (Pathways DC) is an innovative and nationally recognized nonprofit organization committed to ending homelessness for individuals with serious mental illnesses, co-occurring substance abuse disorders and/or other medical challenges in our Nation's capital. Pathways is one of the originators of “Housing First”, in which services are provided to those in need without conditions and pre-housing requirements such as curfews, mandated sobriety or compliance with medication.

The Registered Nurse (RN) is an integral part of the trans-disciplinary Assertive Community Treatment (ACT) team, providing primary health care to consumers in order to promote wellness, recovery and rehabilitation. Responsibilities are implemented within the framework of agency policy, professional ethics, legal scope of practice and regulatory guidelines. Services are provided both at Pathways offices and in the community.

RESPONSIBILITIES:

●     Provide comprehensive and holistic primary care, including: physical exams, development of diagnosis, management of acute and chronic illnesses;

●     Evaluate outcomes of care and make medical recommendations;

●     Evaluate pharmacological treatment regime;

●     Collaborates with other health care providers and makes appropriate referrals;

●     Develop, Maintain and Update plans of care and health care records and other documentation as required;

●     Monthly Medication/Treatment review of team charts

●     Teach health promotion to consumers and staff;

●     Advocate for consumers in accessing and navigating the health care system;

●     Work closely with the Team and consumer in establishing and following the optimal treatment/service plan;

●     Participate in check-in meeting and case reviews, and consults with team members as needed;

●     Collaborate with staff to coordinate prescriptions and orders, and to oversee the overall health of consumer;

●     Establish a work schedule, in collaboration with team leader, to ensure that consumers are seen in a timely manner;

●     Engage qualified consumers in medical services, both on site and in the community;

●     Educate/ assist agency staff to develop increase awareness of community health care agencies /resources;

●     Serve as health care resources to consumers and families/significant others;

●     Participate in facilitating appropriate hospitalizations (medical and psychiatric), crisis interventions and referrals;

●     Communicate with hospital staff to ensure optimal treatment and discharge planning;

●     Establish and lead education and support groups on health related topics, in the health and fitness initiative, e;g; mental illness, psychiatric mediations, nutrition, safe sex , obesity and smoking cessation;

●     Provide clinical supervision of the LPN- Service Coordinator on the team;

●     Generate 80 units of Medicaid billable time each week;

●     Provide in-services and trainings to staff;

●     Perform other related work as assigned;

●     Serve in on-call rotation;


QUALIFICATIONS:

●     Licensed as Registered Nurse in Washington DC;

●     RN degree

●     BS or MS in nursing preferred

●     Certification by the American Nurses Credentialing Center, preferred.

●     Experience or training in community mental health preferred;

●     Experience with dually diagnosed population preferred;

●     Patience, creativity, flexibility, compassion and sensitivity to persons with disabilities and other minority populations;

●     Excellent written and verbal communication skills;

●     Valid Driver’s license and access to motor vehicle required.
