The Case Manager works directly with Empower YOUth clients to prepare them for entry into the workforce and the building of successful careers. The Case Manager will guide clients along their selected Learning Pathways to become work-ready, providing opportunities for exploration of career interests, occupational skills trainings, participation in work experiences. This position will provide group and one-on-one case management to ensure that all barriers to youth success are addressed.

Primary Job Functions:

·      Conduct initial WIOA eligibility screening to assess client eligibility

·      Conduct EarnBenefits screening to connect client to additional resources

·      Administer Empower YOUth Program intake and assessments; collect necessary documentation

·      Secure appropriate support services to overcome any barriers

·      Review assessments and conduct Work Readiness Evaluation

·      Submit all required forms and documents to WIN YDS to verify client eligibility

·      Conduct orientation session at designated Seedco central office for all WIOA-eligible clients

·      Develop goals and Individual Service Strategy (ISS) with WIOA-eligible client; identify appropriate Learning Pathway

·      Ensure client participation in designated program activities according to his/her Learning Pathway and ISS

·      Collect necessary data and documentation to support client outcomes

·      Enter and update client information weekly in the appropriate databases (e.g.

·      Maintain active, regular communication with all assigned clients

·      Coordinate with Mentoring Coordinator to ensure clients are connected to an adult mentor as needed

·      Monitor client education and training progress; provide ongoing support to assist client in reaching ISS goals

·      Provide feedback on performance issues related to client and document retention solutions that were provided

·      Participate in case conference meetings with client, school staff and WIN to ensure client success

Interacts with:          WIN staff, Seedco management and staff, clients

Qualifications –

Education and Related Work Experience:

·      Bachelor’s degree from an accredited college or university (license in social work or counseling preferred)

·      Experience equivalent to 3 years of professional social service work

·      At least 1-3 years of experience working with non-profit organizations, community groups, and/or government programs and experience working with youth and/or young adults from underserved communities

·      Experience in coaching, goal-setting and devising service plans in collaboration with youth

·      Excellent presentation and facilitation skills with the ability to engage groups effectively

·      Ability to engage multiple stakeholders (family, program staff) to support youth program success

·      Ability to manage multiple assignments and flexibility to work in a dynamic, fast-paced environment

·      Experience working in a data driven environment and demonstrates strong computer/database skills

·      Experience working with youth trying to build skills and achieve employment

Knowledge, Skills and Abilities:

·      Knowledge of Positive Youth Development, Motivational Interviewing

·      Knowledge of workforce development

·      Knowledge of school and community-based resources

·      Ability to serve youth with special needs (i.e., pregnant or parenting youth, youth with disabilities, youth with criminal backgrounds, homeless youth) in a non-stigmatizing, culturally-sensitive manner

·      Ability to work flexible hours if necessary

·      Must demonstrate excellent verbal and written communication skills

·      Proficient in Microsoft Office tools
