Full Time Center of Excellence Care Coordinator

•We are seeking case manager candidates with experience working with individuals with opiate use disorders. Prior experience with D&A case management or as a certified recovery specialist preferred.

•JOB SUMMARY:

It is the role of the Care Coordinator to work with individuals with opioid addiction to help each client served access timely services, resources and supports assessed as needed to decrease substance use and relapse. The Care Coordinator will serve as a liaison with providers and service systems in the community. The Care Coordinator will bridge services, collaborate with treatment providers and work with families/supports to enhance recovery.

The Care Coordinator may assist with accessing and transitioning to varying levels of care including detox, rehab, and intensive outpatient. Care Coordinators will work with providers of Medication Assisted Treatment and psychiatric medication. It will be the role of the Care Coordinator to help clients access resources and supports needed such as Medicaid, Social Security, employment, housing, support groups, legal aid, etc. Care Coordinators will support and coordinate the care of COE clients through assessment to aftercare to enhance recovery.

Regular evening, and some weekend and on-call hours are required.

•RESPONSIBILITIES:

1. Meet with prospective COE clients at locations and times conducive to timely engagement with the COE. Care Coordinators may engage prospective clients in hospitals or other treatment facilities, in their places of residence, in prison, etc. Meeting times may include day, evening and weekend hours. Meeting locations and times are based on the most effective way to connect to the client to the COE so he or she is not lost to care.

2. Work with providers to determine needs; follow provider recommendations for treatment including discharge, treatment, screens and aftercare indicated. Review referral materials and complete timely referrals for treatment or services indicated. Participate in meetings with service providers and clients

3. Assist clients with implementing the Service Plan by helping clients access needed services, coordinating multiple services, monitoring and following-up with the client and service providers, and by case advocacy when necessary. Provide transportation as required.

4. Respond to crisis situations when possible.

5. Complete and maintain orderly, legible, timely records of services and other documents in narrative or data formats as required by the agency or its contracts.

WORK AUTHORIZATION/SECURITY CLEARANCE

Candidate must be able to pass the following:

o Drug Screen

o FBI/Fingerprinting Clearance

o Pennsylvania Criminal Clearance

o Pennsylvania Child Abuse Clearance
