The Brookline Center for Community Mental Health seeks a Case Manager to participate in a groundbreaking program directed at improving the care of adults with a combination of serious mental illness and chronic medical conditions. The program, called Healthy Lives, reaches out to individuals in their homes, providing therapeutically-informed care coordination, education, and support. The goal is to increase access to medical and mental health care, decrease avoidable hospitalizations, and improve mental and physical wellness and self-care in a group of patients who face multiple barriers to getting adequate medical care. The program has demonstrated success working with a diverse array of medical providers in multiple Boston area health systems, including a collaboration with a large accountable care organization.
The Case Manager, a vital member of our team, will work directly with patients in Brookline, Boston, and the western suburbs. Duties include home visiting, accompanying patients to medical appointments, and assisting with connecting patients to social/support services. The position presents an excellent opportunity for an astute, creative, patient-focused individual to help in the development of the program and to care for complex patients in a community setting. The position includes individualized supervision and structured training opportunities to foster professional growth and exposure to a variety of settings, professions, and policy-level discussions in the healthcare field, depending on the professional interest of the individual.
Health outreach, home visits, and accompanying patients to medical appointments.
Supporting and educating patients in order to maintain and improve self-management skills.
Care coordination/case management, including identifying and connecting clients with services and resources.
Actively communicating with patients and their care teams through face-to-face, telephonic, and digital contact.
Maintaining accurate, timely documentation in program databases and electronic medical records.
Participation in program evaluation, including individual and programmatic review, participation in case consultations, and active involvement in the collection and interpretation of health outcome measures.
Maintaining a constructive work environment and effective communication with other employees and managers.
Maintaining confidentiality of client and employee information.
Minimum of a Bachelor's Degree in a related field.
Minimum of 1-2 years of experience during college or post-graduate in a medical or mental health setting.
Comfort in coordinating the support and management of patients with complex psychiatric conditions.
Able to both collaborate closely with an integrated team and to work independently.
The proven ability to prioritize and flexibly manage a variable workload.
A strong interest to pursue a career in a related field.
Excellent oral and written communication skills.
Experience and proficiency with Microsoft Office and online record keeping.
A valid driver's license, access to a working vehicle, and ability to drive to a variety of locations within metro Boston on a daily basis.
Physical abilities: able to climb stairs and visual/hearing ability sufficient to comprehend written/verbal communication.
40 hours/week, 9am-5pm, Monday-Friday.
The Case Manager will receive individual and group supervision, as well as access to comprehensive, Center-wide continuing education programs, and will participate in team and program meetings.
