Clinical Case Manager (LCSW/LPC)

Position Summary 

The Clinical Support Services Case Manager will provide clinical case management and referral services to Foundation Communities residents, who are exhibiting significant social, emotional, or behavioral difficulties or who are experiencing trauma in their lives due to mental health issues, substance abuse, medical issues, CPS involvement, or other significant stressors.

Key Responsibilities

• Provide clinical case management and/or referrals to residents identified by Foundation Communities staff as having significant social emotional or behavioral issues or who are experiencing family trauma.
• Conduct client screenings, including quantitative and qualitative assessments.
• Develop and review service plans in consultation with clients, conduct follow-ups assessing the quantity and quality of services provided.
• Provide mental health consultation and guidance to property and Learning center staff, in matters of de-escalation, healthy boundaries, and trauma responsive care.
• Collaborate with property and Learning Center staff to provide support for residents and meet ongoing needs through existing Foundation Communities programs as well as community resources.
•  Adhere to agency Policy and Procedure
• Attend all required agency meetings and trainings including department staff meetings, team meetings, clinical meetings, all staff meetings and clinical training workshops.
• Maintain documentation in accordance with professional standards in a timely manner.
• Utilize skills in mental health assessment, diagnosis, and treatment planning.
• Utilize biopsychosocial evaluations, trauma screenings and assessments, psychiatric evaluations, risk assessment within scope of licensure and professional competence.
• Utilize knowledge of principles, methods, techniques and evidence-based practices in counseling to support case management work.
• Meet regularly with participating individuals to identify areas of need, employ crisis intervention and de-escalation strategies, and evaluate progress toward improving personal health and stability.
• Provide individual, family and group counseling as needed.

Other Duties & Responsibilities:

• Assistance with maintaining client registries
• Assistance with resource dashboards and referral information

Education and Experience Requirements

• Master’s Degree from an accredited college or university in social work, counseling, psychology, or related field.
• Licensure as a LCSW and/or LPC in the state of Texas with the ability to be credentialed and bill for mental health services in State, Federal, and Private payer programs
• A minimum of one (2) years of experience working in a mental health setting providing trauma responsive services to ethnically and socio economically diverse and/or homeless populations.
• Experience with severe mental illness preferred.
• Proven ability to work independently and as part of a multidisciplinary team.

Physical Demands/Work Environment: Multifamily housing community with significant challenges related to housing instability and socioeconomic stressors. Must be able to work with diverse population, challenging work environment with at risk youth and families.
