Caseworker

GENERAL DESCRIPTION:

This position is responsible for providing intake, assessments, case management and case recording, and crisis intervention for blind and visually impaired clients.  

ESSENTIAL FUNCTIONS:

1. Conduct new client intakes and assessments of client needs and determine supports required due to vision impairment.

2. Determine client eligibility for BCABVI programs/services as set by established standards.

3. Develop an individual service plan in conjunction with client including goals, timelines, agency responsibilites and referrals to other agencies as appropriate.

4. Complete annual reviews/satisfaction surveys for active clients 

5. Facilitate client support groups and activities in the center and community

6. Coordinates with Low Vision Clinic for services, information and assistance with devices used by clients and information about other useful tools to enhance client’s quality of life.

7. Reviews and evaluates available resources and services from local agencies to provide social assistance for clients.

8. Schedules appointments in the office and at client’s home – (travel within county required)

9. Maintain regular communication with active clients and document interactions in client database. 

10. Acts as advocate for clients

11. Maintains confidentiality of all client information

12. Attends staff meetings, training sessions and conferences as assigned 

13. Prepares monthly reports as assigned


EDUCATION, EXPERIENCE AND SKILLS REQUIRED:

Bachelors Degree in human services field. Advanced degree preferred. Experience in case management required.  

Working knowledge of Microsoft Office programs, Google Workspace and ability to perform data entry for the purpose of maintaining client records.
