Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking qualified candidates for the Lead Mental Health Professional to work with our Intensive Care Case Management (ICCM) team in Philadelphia, PA. This position provides clinical supervision in assessment and treatment planning, and in the delivery of treatment, rehabilitation and support services, and provides support and back-up to the ICCM Director. This member of the multidisciplinary team has training in one of the mental professions (e.g., nursing, social work, rehabilitation counseling or psychology) and clinical skill and experience to assess, plan, develop, coordinate and provide treatment, rehabilitation, and support services to program participants with severe and persistent mental illness under the clinical supervision of the ICCM Director and the Psychiatrist. Some other responsibilities would include:

·      Provide service coordination (case management for an assigned group of participants including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the participant and the individual treatment team (ITT); provide individual supportive therapy, psychotherapy, and symptom management, ensuring immediate changes are made in the treatment plans as participants’ needs change; educate and support participants’ families, and advocate for participants’ rights and preferences.


·      Conduct comprehensive assessment of psychiatric history (e.g. onset, course an effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment, social development and functioning; activities of daily living (e.g., care-care, living situation, nutrition, money management); and family structure and relationships.


·      Acts as liaison and consultant with community agencies and families to maintain coordination in the treatment process.


·      Provide ongoing assessment of participants’ mental symptoms and participants’ response to treatment. Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior that put participants at risk (e.g., suicidal).


·      Coordinate with outside inpatient services to detoxify participants and establish linkage to outpatient treatment, self-help programs (e.g., Alcoholics Anonymous, Narcotics Anonymous), outpatient services, and residential facilities.


·      Conduct on-the-job performance assessments and evaluations, regular work review sessions with participants and their employers, on-the-job support, and crisis-assistance contacts.


·      Assist participants to find and maintain a safe and affordable place to live: apartment hunting, finding a roommate, landlord negotiations, cleaning, furnishing and decorating, and procuring necessities (e.g., telephone, furnishings, and linens.)


·      Teach money-management skills and assist participants in accessing financial services. Help participants to access reliable transportation. Assist and support participants to have and effectively use a personal primary care physician, dentist, and other medical specialists as required.


QUALIFICATIONS:

·      Master’s degrees in human services, nursing, social work, rehabilitation counseling or psychology.

·  Must have a strong commitment to supporting the rights and abilities of participants with severe and persistent mental illness to live in a normal community residence; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.

·  Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preference in treatment are essential.

·  Must have a valid driver’s license with an acceptable driving record for the state in which the candidate lives.


SCOPE:

·      Frequent talking listening.

·      Requires the ability to lift, carry, hold, kneel, reach, stack, stoop and bend, which may include direct intervention with participants.

·      Possess manual dexterity and fine motor skills.

·      Local travel required, including transporting program participants.

·      Requires crisis intervention in emergency situations.

·      May be required to report to work during emergencies, including inclement weather.
