New Alternatives for Children, Inc. (NAC) is an innovative child welfare agency located in Midtown Manhattan, which provides services to children with medical complexities, intellectual disabilities, mental/behavioral health challenges and their families. NAC is currently seeking applicants for a Social Worker to work in the Foster Care and Adoption Department.

Disclaimer: “At this time, while this position requires some in-person field work, a good portion of the work is conducted at home remotely. The staff will be afforded a laptop to participate in required meetings and trainings and as needed, contact with families.”

Responsibilities:

·      Provides case planning activities for children placed in NAC’s Special Medical and Treatment Family Foster program.

·      Conducts needs assessment for children and their original caregivers to ensure linkage and monitoring of service participation to promote positive behavior change.

·      Participates in the development of the treatment plan for each child on his/her caseload in collaboration with an interdisciplinary team.

·      Provides technical and emotional support to the foster parent including the implementation of specific strategies and interventions, assessing of problems, and behavioral analysis.

·      Responsible for on-going assessment of progress toward the treatment plan through client contacts and team meetings with the behavioral specialist, and or assigned registered nurse and supervisor.

·      Responsible for case management as per ACS policies and guidelines toward permanency.

·      Coordinates and provides services to the birth family. Meets with service providers to assess client progress.

·      Attends Family Court hearings, Family Team Conferences, Treatment Team and Interdisciplinary Conferences.

·      Maintains accurate and timely record keeping.

·      Conducts face to face contact with foster parent and child in the foster home, birth family in the home and community, and all required collateral contacts with providers. Provides support and supervision during family visits.

·      Maintains regular phone contact with the foster parent.


Minimum Requirements:

·      MSW from an accredited school of social work; LMSW preferred or Masters in Mental Health

·      Prior direct experience working with youth and their families in child welfare, mental health, medical or an educational setting

·      Experience in formal or informal therapeutic relationships with youths is desirable

·      Knowledge of health-related factors in working with physically disabled and/or chronically ill children who have psychiatric diagnoses and intellectual/developmental disabilities

·      Excellent interpersonal communication, writing and organizational skills are essential

·      Flexibility to work both remotely and in-person
