Description

University Behavioral Associates (UBA) at Montefiore Medical Center in Bronx, New York is seeking a dedicated and organized bachelors- or masters-level individual with experience in social services to serve as a case planner for the Court Ordered Supervision (COS) program funded by the NYC Administration for Children’s Services (ACS).

 

The COS program is a community based, family centered program that works closely with parents, children, and providers in order to ensure a strong, effective collaboration that supports families in focusing on child safety and well-being, as well as improved family functioning.



Case planners will be responsible for working closely with families dually involved with the child welfare and legal systems because of mental health or substance abuse concerns. The case planner will conduct ongoing in-home assessments of family members, make referrals to appropriate clinical services, and monitor attendance and engagement at treatment programs.The case planner will also be responsible for ensuring coordination between families, providers, and the legal system.

 

The ideal candidate would be organized and strengths-based, with experience working with families and coordinating services, as well as comfort interacting with the Court system and in-home services. Evening and/or weekend availability may be required at times.
