PURPOSE OF POSITION: The incumbent provides linkages, advocacy and other supports for program participants assigned to their caseload.

 

RESPONSIBLE TO:            Program Manager

 

SUMMARY OF DUTIES:  Responsibilities to include but not limited to the following:

 

•            Establish a positive rapport with program participants while maintaining professional boundaries.

•            Assists residents in a manner that demonstrates respect, honesty, empathy and functional knowledge of                the primary components of trauma informed care.

•            Serves as a positive role model for program participants by demonstrating good work ethics.

•            Coordinate and provide agency education and employment support, resources and activities for NEW                clients and staff. 

·               Collect, compile and maintain agency education and employment client statistical data.

•            Develops an Individual Service Plan for each newly assigned program participant within (3) days of admission and updates the service plan at a minimum of every 90 days.

•            Creates case files for each assigned program participant and updates files as required.

•            Meets with program participants assigned to caseload a minimum of twice per month, based on individual needs. Performs and completes monthly Participant and Housing Assessment reports during home visits as required by New Horizons.

•            Provides an orientation for program participants newly assigned to case load.

•            Meets with program participants assigned to caseload weekly and/or bi-weekly based on individual client needs and mandates of program contract.

•            Documents case management meetings and other pertinent information in case files to ensure they accurately reflect program participant’s progress, concerns and service delivery.

•            Provides coordination and referral assistance to program participants in accessing community support services and entitlement benefits. Support services include but are not limited to GED preparation, job training, housing assistance, primary medical care, substance abuse treatment and AA/NA meetings.

•            Compile and enter universal and program specific data elements as defined in HUD’S Final Notice on       HMIS (Homeless Management and Information Systems Database) Data Standards.

•            Review, explain and provide a copy of the New Transitions Universal and Additional Program rules and procedures to all participants assigned to case load.

•            Review, explain and provide a copy of the Homeless Services Reform Act of 2005 (HSRA) and Reasonable                Accommodations to participants assigned to your case load.

·               Provides monthly statistical data to program manager for all New Transitions.

•            Responsible for participant entry process (observing unpacking, completing NEBW property check off list, issuing of keys, providing information and completing required program entry documents .

•            Responsible for the total participant exit process (observing packing, completing NEBW property check  off list, recovery of keys, conducting exit interview.

•            Conducts random urinalysis screenings of program participants as stipulated in the client service plan      with follow-up action plans as needed. 

•            Reports programmatic needs or concerns to the Program Manager.

•            Attends all required trainings and provides verifying certificates to supervisor for submittal to     employee file.

•            May have on-call responsibilities as needed for program crisis support and intervention.

•            Perform other duties as assigned including but not limited to receptionist desk support as needed.

 

SUPERVISORY DUTIES:  This position does not supervise any other staff.

 

WORKING RELATIONSHIPS: Regular internal contact with NEW staff, regular contact with other agencies.  Functions as a team player, acts responsibly, develops mutual respect and has an appreciation for diversity.

 

KNOWLEDGE AND SKILLS:

 

Education:          Preferred B.A./B.S. Degree in Social Work/ Psychology

 

Experience:       Minimum of two years’ experience in human services

 

Knowledge/Skills:           Strong written and verbal communication, organizational and interpersonal skills.                                         Commitment and interest in issues affecting the homeless, low income and underserved populations.                                                                                                                                   Demonstrates initiative and strong leadership skills                                                                        Computer literacy, proficient in Windows environment

                                               Functional knowledge of HMIS database

                                               Trauma Informed Care and Motivational Interviewing

                                               Workforce Development                             

 

Work Environment / Physical Demands:

 

•            Sustained concentration

•            Frequent interruptions

•            Moderate noise level

•            Occasional travel between office and community providers for outside meetings, trainings and resource                access
