Merrimack Valley Assistance Program (MVAP) is a long-running HIV/AIDS Service Organization. Established in 1995, MVAP is a non-profit and state-licensed case management agency, providing case management, supportive services, housing assistance, and referrals to persons living with HIV/AIDS.

Our Mission: to provide, or help gain access to, essential services such as safe and affordable housing, adequate food and nutrition supplements, proper medical and dental care, and behavioral health services, as well as to provide education to the community-at-large.

About the Position: A Case Manager at Merrimack Valley Assistance Program (MVAP) will be in charge of providing case management, harm reduction counseling, and supportive services and referrals to agency-eligible individuals. All agency-eligible individuals are persons living with HIV/AIDS and are earning less than 500% of the Federal Poverty Level (FPL), and are part of the Ryan White CARE Program. As a Case Manager, you will ensure that clients remain enrolled with MVAP and the Ryan White CARE Program, providing them access to medical care, including a PCP and/or HIV Specialist, safe and stable housing, and food and nutritional services. The Case Manager will determine client eligibility of these programs and their services including housing assistance, prescription medication coverage, mental health services, and substances use disorder treatments. The Case Manager will help clients to access these various programs and services in the community, for which they may be eligible for due to their HIV/AIDS or Hepatitis-C status.

Please note: All case manager positions are funded (at least in part) by various government grants and contracts. While it is anticipated that these grants and contracts will continue to be renewed and that funding for these positions will continue, this is not guaranteed.

Responsibilities of Position:

· Determine client eligibility for MVAP and the Ryan White CARE Program and their services.

· Provide risk reduction counseling in relation to sexual health, behavioral health, and substance use.

· Assess the needs of clients, with special attention to the basic needs for food, safe and stable housing, and appropriate health care.

· Develop goals (or Individual Care Plans) with clients ensuring that all obstacles and strategies on how to achieve them are identified.

· Provide information to clients about housing options, community social services and/or government agencies and their respective programs, services, or benefits for which they may be eligible.

· Assist clients in making inquiries about and/or applying for adequate housing, applicable programs, services, or benefits.

· Document, monitor, and evaluate all encounters with clients.

· Work collaboratively with the staff of other agencies to insure a coherent and coordinated delivery of services to clients.

· Perform such other duties in the service of clients as may be requested by the Director of Office Operations or designee.

Requirements:

· Bachelor's Degree in an appropriate field (such as a social science, a health science, or a applicable field sufficient to perform job duties (preferred) or years of work experience working with in public health, sexual and behavioral health, risk reduction counseling, and/or nonprofits.

· Knowledge of sexual health topics including; HIV/AIDS, hepatitis C, sexually transmitted infections (STIs), PrEP, modes of transmission, and preventative strategies.

· Ability to work in a culturally diverse environment, with clients and MVAP staff, where ALL individuals are accepted and welcomed. These communities include, but is not limited to; LGBTQ+, Refugee, Immigrant, Undocumented, BIPOC, etc.

· Knowledge of community programs including food pantries, health care facilities, substance abuse services, and supportive groups and programs.

· Experience talking with individuals one-on-one about sensitive and/or private information including demographics, medical history, current health status, income, and life experiences.

· Excellent verbal and written communication skills.

· Ability to work collaboratively with staff members from various agencies, but also independently with each of your clients.

· Experience working with various computer programs and applications, to carry out day-to-day tasks and duties, with Microsoft Word, Excel and Outlook, and the ability to learn new programs, applications and databases, utilized by MVAP.

· Possession of a valid driver's license and current automobile insurance coverage, including personal injury and medical coverage. Proof of both will be provided to one's supervisor annually.

· Ability to speak and understand Spanish, or any other language represented among the clients of MVAP, is desired.
