    Job Title: Social Care Network (SCN) Community Liaison  

    Reports To: SCN Project Director 

    Project Summary: 

    The New York State Department of Healthhas established Social Care Networks (SCNs) as part of the 1115 Waiver Demonstration Amendment, and Forward Leading IPA (FLIPA) is the SCN Lead for the Finger Lakes Region. This region is comprised of 14 counties, including Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, and Yates. The SCN’s aim is to enhance the delivery of social care services to Medicaid members by coordinating efforts among community-based organizations (CBOs) and other health care partners to create a more resilient, flexible, and accessible social care system that reduces health disparities and advances health equity. The SCN will collaborate with CBOs and other health care partners, leveraging shared data and technology to coordinate social care services for Medicaid members to improve access, ensure reliable and timely referrals, streamline and track navigation and completed referrals in closed loop systems and enhance collaboration between social care service providers and other regional partners. 

    Organizational Overview:  

    Forward Leading IPA (FLIPA) is a nonprofit membership association of safety net providers working in partnership to provide the highest quality integrated healthcare to historically underserved populations in Upstate New York since 2017. FLIPA is renowned for its commitment to integrating primary care, behavioral health, and social care needs. Our growing membership serves individuals across more than 27 counties and includes federally qualified health centers (FQHCs), behavioral health providers, and a rural health network consisting of eight county public health departments.  

    Job Summary 

    The SCN Community Liaison engages stakeholders and ensures FLIPA SCN activities and leadership are informed by meaningful feedback and needs identified within the communities served. The SCN Community Liaison serves as a communication bridge between communities, community initiatives, SCN activities and state-required deliverables. This position is grant-funded through March 2027. 

    Duties/ Responsibilities: 

    • Support the SCN Project Director to engage stakeholders in activities of the SCN  
    • Build relationships within the Finger Lakes region with community stakeholders including but limited to community-based organizations (CBOs), health systems, relevant community initiatives and projects, local government units (LGUs), departments of social services (DSSs), and community members receiving Medicaid 
    • Represent the SCN at community meetings and events  
    • Provide information to the community related to the activities of the SCN  
    • Develop and implement engagement activities such as topic specific work groups and network-building activities, ongoing learning collaboratives, and engagement in HRSN activities 
    • Participate in development of messaging and materials for community education about SCN activities 
    • Host workshops and events for CBOs to learn about participation in the SCN 
    • Provide logistic support for engagement activities including meeting planning, communication and other support as required 
    • Ensure engagement of grassroots CBOs and Medicaid members most likely to experience inequities 
    • Provide feedback and guidance to FLIPA SCN leadership to ensure these perspectives are included in the activities and planning of the SCN 
    • Provide support for potential CBOs to develop readiness for contracting with the SCN and/or provide handoff of CBOs to Partner Liaison Specialists for readiness support  
    • Identify challenges and opportunities for improvement related to implementation of assigned activities 
    • Effectively work across multiple organizations and geographies to support project progress within different organizational culture structures and communities 
    • Other duties as assigned 

    Education & Experience: 

    • Bachelor’s degree in related field, such as social work, community development, public health, healthcare administration or equivalent experience preferred but not always required. Relevant work experience may be considered in lieu of a degree. 
    • Demonstrated experience connecting with community leaders, organizations, and resources in the identified SCN region 
    • Experience with grants/projects within the health care sector in New York State (population health, behavioral health, Federally Qualified Health Centers, or similar) preferred 
    • Familiarity with the local community, its demographics, and social and economic factors. Prior experience with large-scale implementation grants/projects preferred 
    • Proficiency in one or more languages other than English prevalent in the Finger Lakes region is a plus

    • Sensitivity to cultural diversity and an understanding of how cultural factors may impact community dynamics and interactions 
    • Familiarity with regional CBOs, partners, communities, and community initiatives preferred 
    • Excellent organizational skills 
    • Ability to independently manage multiple priorities effectively  
    • Strong interpersonal skills, independently resourceful and thrives in a fully remote environment 
    • Strong problem-solving skills and the ability to identify creative solutions 
    • Proven ability to complete projects according to defined timelines and objectives 
    • Proficient with MS Office and various business software 

    Salary Range: 

    • Salary is commensurate to education and experience with range of $45-55k. 

    Additional information: 

    • This position will be primarily remote. However, given the nature of the role, approximately 50% of the workday is anticipated to include regular travel and in-community work within the Finger Lakes region for community convenings and relationship building. Occasional travel across New York State is also expected for on-site meetings or training. 
    • Residency within the Finger Lakes region of New York State is required.
