POSITION DESCRIPTION

Working in the Community Initiatives team is a rewarding challenge. Every day is different and projects are constantly starting, evolving, and closing out. The environment is fast paced yet thoughtful, and there is ample room for creativity and independence, although most projects are carried out with team support.

The main focus of the Health Equity Coordinator is to lead efforts that advance the health equity goals of the communities we serve. This role will manage the Greater Northside Health Collaborative and the Accountable Communities for Health project. This position brings together efforts across the communities we work in, and between the different plans and community teams. The position will work closely with out CI Project Manager, and Community coordinators.
