Position Title:    Community Health Contact Tracer 

Reports To:         Manager, Education and Wellness

Classification:    Full time,-Non-Exempt, 1.0 FTE, temporary through the end of December 2021 (possible extension dependent on available funding)     

Compensation:    $39,520-$45,760/year ($19-22/hour)

Schedule:              Must be available to work weekends (Saturdays and Sundays)

POSITION DESCRIPTION

The Latinx community has been hit disproportionately by the coronavirus. While only 13.4% of Oregon’s population identifies as Hispanic or Latino/a/x, 39% of people with positive COVID-19 tests in Oregon have been identified as Hispanic. To suppress COVID disease transmission, it is critical to identify as many people who have been exposed to the virus as possible and then conduct broad Contact Tracing to identify all Contacts that may have been exposed to a case during their infectious period. Community Health Contract Tracers who have similar training or skill sets are trusted community members who can effectively build relationships and make Contact Tracing successful. This trusting relationship enables CHCTs to serve as a liaison/link/intermediary between local public health authorities and the community to facilitate access to services and improve the quality and cultural competence of service delivery, as well as ensure individuals impacted by COVID-19 can successfully complete Quarantine or Isolation. 

The Community Health Contract Tracer works directly with clients, families, and health care personnel to support the overall health and well-being of the Latinx community as it relates to the current Covid-19 pandemic. This position will be working with community members who have tested positive for COVID-19 within Multnomah, Washington and Clackamas Counties.


This position requires a bilingual, flexible, self-directed person, with bi-cultural proficiency who is excellent at navigating various high-stake community health related situations. They must have strong skills including initiative and drive to search and resource information as needed. This person should have experience with conducting home visits and accompanying people to medical visits. The CHCT must be able to empower community members to manage their health and to face difficult health issues. Experience and knowledge of the health care system and social service resource navigation is required. 


RESPONSIBILITIES:

The CHCT will provide the following services to individuals, families, and other community household members who test positive for COVID-19, including, but not limited to:

  • Contact Tracing:

             o   Work directly with Washington and Clackamas County local public health authorities (LPHAs) and the Oregon Health Authority to receive information about Contacts needing to be traced. 


              o  Contact Tracing includes: (1) Contacting all identified Contacts to document a symptom check; (2) Referring.          Contacts for testing according to established protocols; (3) Providing Contacts with Quarantine instructions; and (4) Documenting work in the OHA ARIAS system.

              o Utilize Community Health Worker practice and methodology to establish trust with Contacts, assess and collect information required by OHA, and follow scripts, policies and procedures provided by OHA or LPHAs. 

              o Comply with all state and federal HIPAA and other privacy/confidentiality requirements and current investigative guidelines.

  •  Wraparound supports for individuals who test positive or are close contacts:

o Provide culturally centered education on COVID symptoms monitoring and prevention.

o Provide informal counseling and emotional support to cope with social isolation.

o Support navigation and advocacy for various essential social services and resources (e.g., access to food, housing support).

o Provide additional support/education for individuals/families with chronic conditions.

o Support/coordinate care with health systems.

  • Outreach and Education:

o Educate the community about safe practices to prevent the spread of COVID-19, testing, and other COVID-19 resources through virtual channels and act as liaison between the community and state and local public health authorities 


Additional Duties:

• Support Latino Network community testing efforts.

• Enter client/referral data into Latino Network’s Efforts to Outcomes database, and other databases when necessary.

• Coordinate and facilitate other health promotion and health education related classes when needed.

• Participate in health fairs, including some nights or weekends.

• Write monthly reports.

• Communicate the progress on all tasks and relay any urgent information in a timely manner to Manager and Department Director.

• Maintain punctual, regular and predictable attendance.

• Work collaboratively in a team environment with a spirit of cooperation.

• Respectfully take direction from supervisor.

• Other duties as assigned.


Requirements

REQUIRED SKILLS/KNOWLEDGE/ABILITIES: 

The requirements listed below are representative of the knowledge, skill, and/or ability required for this position.

  • Bachelor degree or 3 years of equivalent work in health clinic, social work, or community health work.
  • Certification as a Contract Tracer or Community Health Worker preferred, but not required.
  • Minimum of three years of experience in community outreach services providing health information, advocacy, social support and assistance in using the health care system to groups and families; AND demonstrated experience working with the Latinx community within the medical system.
  • Bilingual in English and Spanish with the ability to communicate effectively, both orally and in writing. Additionally, Indigenous languages from Latin America a plus.
  • Understanding of medical terminology and ability to convey the concepts to clients in Spanish.
  • Ability to maintain strict confidentiality and follow HIPAA policies and procedures. 
  • Bicultural proficiency required: knowledge of Latinx culturally based world view, adaptive reasoning, and problem-solving practices of Latinx individuals, families, and community living in urban neighborhoods impacted by crime, poverty, and racial inequity. 
  • Self-motivation and the ability to work independently with strong analytical and problem-solving skills.
  • Possess excellent attention to detail, great organizational skills, and ability to prioritize multiple tasks.
  • Excellent working knowledge of Microsoft Office, Google suite, and data entry skills.
  • Excellent phone and computer skills are required.
  • Reliable transportation, valid driver license, and automobile liability insurance required; eligibility to be an insured driver under Latino Network’s Commercial Auto Liability preferred.

The employee may be required to perform walking during peak events and minimal heavy lifting - must be able to regularly lift and /or move up to 25 pounds, frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds.


WORK ENVIRONMENT

  •  The majority of the position will happen through telework (video conferencing and telephone).
  • Frequent use of keyboard, monitor, mouse, telephone and/or headset. 
  •  This role will also periodically deliver materials (using physical distancing) to the households who have tested positive for COVID-19.
  •  Because the work will be responsive to the number of COVID-19 cases at any given time, workload may fluctuate. 
  •  This position requires a flexible schedule to work evenings and weekends in a fast-paced, highly interactive, and time sensitive needs environment.
  •  While performing the duties of this job, the employee is occasionally exposed to wet or humid conditions (non-weather); outdoor weather conditions; extreme cold (non-weather) and extreme heat (non-weather). The noise level in the work environment is usually moderate.
