Position Description: The Lead Case Manager (LCM) will be responsible for service

provision within the Case Management Program for the purpose of helping clients improve

their quality of life and promote self-sufficiency. The LCM must be able to enroll and

maintain a caseload of approximately 20 clients and guide clients annually to

self-sufficiency, in addition to coordinating workshops, providing leadership for resource

counseling, sustaining community partnerships, and maintaining program materials for

outreach efforts. Days and hours of work are Monday - 9:00 am to 5:00 pm; Tuesday,

Wednesday, Thursday, & Friday - 11:00 am to 7:00 pm. Hours may be adjusted as job

duties demand.

Primary Responsibilities:

● Determines client eligibility and appropriateness for services, refers to other

agencies or provides services according to program guidelines.

● Provides case management services in an effort to:

○ Provide work readiness (i.e. job seeking, application process, resume writing,

interview preparation, and job retention skills) and education coaching for

clients.

○ Assist clients with managing goals for financial, education, employment, life

skills, and health and wellness. As such, determine the need for and refer to

appropriate services in areas of education, employment, job training, health

services, credit counseling, child care and transportation.

● Monitors client progress through direct client contact, home visits, phone calls, and

letters and documents progress by maintaining progress notes and goals.

● Responsible for the entry of client data and the use of the data for case management

services and reporting.

● Prepares data and information for periodic reporting to the Executive Director.

● Maintains caseload to meet the outcome requirements of all funding sources and

evaluation partners.

● Responsible for establishing and maintaining collaborative relationships with

community partners.

● Develops and maintains marketing and program materials, with specific attention to

translation of materials for linguistic needs of clients.

● Develops and conducts workshops with a primary focus on work readiness, credit

counseling, life skills, and other self-sufficiency related topics.

● Recruits, screens, provides work coordination, training, and supervision to resource

counselors, student interns, and other volunteers, as assigned.

● Participates in agency planning and coordination with other agency services,

programs, and events, as assigned.

● Ability to communicate with and be sensitive to the needs of a diverse client

population.

● Exhibits excellent leadership skills.

● Ability to focus and prioritize multiple projects simultaneously and meet deadlines.

● Acts professionally and responsibly in all work areas.

● Communicates effectively orally and in writing.

● Other duties as assigned by the Executive Director.

Work Environment: Work is performed in an office and requires driving to multiple service

sites. Continuous contact with other staff and the public is expected.

must be met by an employee to successfully perform the essential functions of this job.

the essential functions. While performing the duties of this job, the employee will regularly

sit for extended periods of time, frequently stand and walk, need normal dexterity and

eye-hand coordination, need corrected hearing and vision to normal range, and be capable

of verbal communication. The employee may need to occasionally lift and/or move up to 25

pounds. The employee will use office equipment including computer, telephone, calculator,

copiers, and other related peripheral equipment such as printers and scanners.

Minimum Requirements: Four-year degree in Social Work (BSW) required. 2 years of

experience required. Must have reliable transportation and be computer proficient and able

to document work in web-based database. Bilingual in English/Spanish required. Able to

affirm Shepherd’s Pantry faith-based mission.

Pay/ Benefits: Pay rate ranges from $19 to $21/ hour based on experience. Benefits

include paid vacation, sick leave, and mileage reimbursement.
