General Responsibilities:

Establish a positive rapport with program participants while maintaining professional boundaries.


Complete intakes for all new youth entering CHGW including an intake assessment, initial

case plan, releases of information, PHQ -9, suicide risk assessments, and safety plans as needed;


Develop and facilitates group life skills workshops weekly


Manage a caseload of 8-10 youth


Helps develop and write, monitor and update youth’s Individual Case Plan in

collaboration with youth, and with external service providers;


Monitor and track progress of the youth with support to stable housing by making the

necessary referrals for housing interventions outside of the transitional living program


Present cases during Coordinated Entry meetings when needed


Meet with the youth weekly to address and monitor progress


Provides case management support to the youth’s inter disciplinary teams which includes

but not limited to identifying and addressing psychosocial concerns, engagement

in services, coordinating care, advocacy, and health related and housing

barriers.


Provides assistance in scheduling appointments and accessing community support services

as needed. Accompanies youth to appointments when necessary.


Participate in weekly case reviews, transition meetings and supervision;


Consults with supervisor on difficult to engage youth


Provide Crisis management support to any youth identified as in crisis in both

residential and at the Service Center.


Clinical On-Call rotation -- Provide Crisis support to youth in all programs when

identified as the on-call clinical support team member. 


Ensure timely and accurate documentation of assessments (e.g. Intake, Initial Case

Plan, TAY), progress notes, case plans, verbal encounters, document needing to

be uploaded and all contractually required documentation in Homeless Management

Information System (HMIS) and Efforts to Outcomes (ETO);



Knowledge, Skills and Abilities

College degree in Human Services, Social Work or Psychology

Some supervisory experience preferred

Is approachable, builds rapport well and practices attentive and active listening;
