Case Manager for Women’s Center

Full Time Exempt                                                                     Salary: Range $45,000 - $50,000

Reports to: Operations and Program Director                  Benefits:  Medical, Vision, Dental, Disability

 

POSITION PURPOSE

This position provides direct, supportive, ongoing coordination of services to assist homeless women at Jubilee Women's Center in obtaining employment, permanent housing, and personal empowerment.  Includes but is not limited to needs assessment, goal planning, resource identification and development, advocacy, service monitoring and follow-up.

DUTIES AND RESPONSIBILITIES

The Case Manager role is to provide support for the residents and be accountable for the effective running of Jubilee’s three locations. This position interacts with residents, staff, donors, volunteers, neighbors and other agencies. 

·      Resident Services

o  Assure basic needs and safety of residents.

o  Commit to providing an inclusive, caring, honest and respectful environment.

o  Manage a caseload of up to 20 residents.

o  Work with residents to develop short and long-term goals and create pathways to   achieve those goals.

o  Assist in the determination of new program participants and residents.

o  Provide crisis management, intervention, problem solving, and conflict mediation, when necessary.

o  Work with community partners to develop referral sources and community resources for Jubilee participants.

o  Assist with Resident House Meetings, tours, community meals, volunteers, and participate in Jubilee events, as needed.

o  Provide follow-up to former residents, as needed.

o  Share in the rotation of the emergency phone and respond to after-hours emergencies, as required.

o  Provide new resident orientation for women moving into Jubilee.

o  Possess a solid knowledge and understanding of the Resident Handbook and Community Code guidelines. Be willing to provide honest, candid feedback and consequences when guidelines are not followed.

o  Treat all participants, staff and volunteers with dignity and respect.

 

·      Administrative Duties

o  Maintain clear, accurate, current notes in resident files of all significant interactions and goal achievements.

o   Facilitate initial assessment of residents and oversee and monitor self-assessments for residents.

o  Maintain up-to-date and accurate records in the Outcome Tracker database.

o  Articulate and follow appropriate standards of professional and personal boundaries.

Minimum Qualifications:

Ø Two years or more of progressive adult case management or similar experience.

Ø Willing to embrace the mission and goals of Jubilee Women’s Center.

Ø Ability to work well under pressure; handle multiple time-sensitive tasks; strong organizational skills.

Ø Sensitivity toward cultural, racial, ethnic, socio-economics, and sexual identity diversity, as well as physical/mental health abilities.

Ø Commitment to excellence and continuous improvement in a rapidly changing environment.

Ø Ability to work flexible hours including some evenings and weekends. 

Ø Ability to work independently and as part of a team, including group activities with residents.

Ø Valid driver’s license, reliable, insured vehicle and ability to use personal car, as needed.
