Description

The MOC Team Medical/Psychiatric Liaison role is to coordinate with and support the activities of the team’s medical providers (including psychiatric providers and Street Medicine Team). One FT staff member serves as a liaison for each catchment area between:

·         providers and case managers

·         providers and team leadership

·         providers and clients

·         the team’s medical and psychiatric providers to deliver integrated care

·         the team’s medical/psychiatric providers and other providers or services in the community

 

The liaison and will work closely  with the Program Director or appropriate supervisor in their collaborative efforts with Housing Advocates/Case Managers/Housing Outreach Specialists to implement the recommendations from medical and psychiatric providers.


Responsibilities include (but are not limited to):


On days psychiatry and Street Medicine Team (SMT) providers are with MOC team:

·         In-person sign in and sign out with SMT and psychiatric providers

·         Ensure Housing Advocates/Case Managers/Housing Outreach Specialists submit refferals

·         Communicate SMT and psychiatric sign out to team at end of day (including recommendations and action items)

·         Assist in looking for clients or escorting clients to team office for scheduled appointments

·         Spend time in the field (either with SMT providers or independently) locating and engaging clients

·         Meet monthly with team leadership and SMT providers to review and update medically vulnerable list

 

Every day:

·         Manage psychiatric and SMT schedule

·         Work with Housing Advocates/Case Managers/Housing Outreach Specialists to follow up on recommendations from psychiatry and SMT (some of the follow up to be done by housing advocates, some to be done by liaison)

·         Coordinate care  with other providers (hospitals, clinics, specialists)

·         Communicate with hospitals and emergency rooms during client acute treatment/hospitalization

·         Communicate with Janian providers on days providers are not with teams about emergencies, need for advocacy, other urgent matters

·         Develop linkages and relationships with providers in the community to facilitate referrals and care coordination

·         Gather medical information received from other appointments or hospital visits

·         Find providers (primary care, specialty, additional health care services) for clients based on provider recommendations, geography, insurance coverage) And assist with insurance authorization process.

·         Scheduling appointments with providers in community

·         Escort clients to medical appointments, labs and diagnostic imaging centers, as needed

·         Consult E-PACES to determine client medical coverage when necessary

·         Track clients’ Medicaid/Medicare certification to ensure recertification and prevent lapses in coverage in collaboration with Housing Advocates/Case Managers/Housing Outreach Specialists

·         Assist SMT with coordination of special projects and public health initiatives such as vaccinations

·         Assist Housing Advocates/Case Managers/Housing Outreach Specialists in obtaining medical records from hospitals and other providers

 

Additional Tasks

·         Weekly supervision with supervisor

·         Monthly meeting of team liaisons (to be determined)

Requirements

·         Must have valid driver’s license.

·         BA plus 2 years direct service experience with indicated populations, BSW and 1 year, or HS diploma and 6 years direct service experience with indicated populations. Note: For every 30 college credits earned, 1 year of experience may be reduced from the requirement for applicants without full degrees.

·         Must be able to work effectively as part of a team.

·         Relevant experience working with population served by the program preferred.

·         Applicants who speak Spanish will be prioritized
