Catholic Charities Neighborhood Services, Inc. (“CCNS”) is a not-for-profit corporation incorporated in 1947. We are a multi service social service agency with approximately 150 programs and services in Brooklyn and Queens. Among the programs operated by CCNS are day care and head start centers, mental health programs, senior centers, AIDS scattered site housing, prevention programs, and a homelessness prevention program. The BHCC- DOHMH program serves and assists individuals with a history of serious mental illness (SMI), and/or a history of multiple inpatient hospitalizations. The primary program goal is to help all service recipients continue living in the community and facilitate access to the services and supports necessary to enhance or maintain their level of functioning and sense of personal satisfaction in their lives and in their communities with respect to their individualized goals and objectives.

CCNS is currently seeking to fill several full time Health Home Clinician position for its Queens Care Coordination program.



Under the direction of the Health Home Team Supervisor for Health Home Care Coordination program, the Health Home Clinician has responsibility for assessing appropriateness and effectiveness of care coordination services, identification of gaps in the service delivery system. Health Home Clinician will actively participate in the development and implementation of processes and services that will improve the service delivery system, outcomes, staff training and development, audit readiness and new initiatives.

The Health Home Clinician will facilitate program’s efforts for patient-centered care for reducing avoidable health care costs, specifically preventable hospital admissions/readmissions and avoidable emergency room visits, providing timely post discharge follow-up and improving health home members’ outcomes by addressing primary medical, specialist and behavioral health care through direct provision, or through contractual arrangements with appropriate service providers, of comprehensive, integrated services.

The Clinical Staff will be required to maintain a professional and ethical disposition at all times, while ensuring that subordinates practice professionalism and ethical behavior as well. The position requires the ability to ensure coverage and supervision at the program level in programs or in situations which require 24 hour/7 day per week response.



  • Master’s degree in a social work or related field required. 
  • Master’s degree may be substituted for a nursing degree and RN License.
  • Position requires one of the following: NYS RN, NP, LMHC, LMSW/LCSW CASAC, License or a Licensed Psychologist.
  • The position requires excellent clinical skills.
  • Must have excellent written and verbal communication skills, comfort with public speaking, lobbying and advocacy.
  • Belief and understanding of the Agency Mission.
