Health Educator Associate

The Mount Sinai Adolescent Health Center (MSAHC) has an opening for a Health Educator Associate to join its highly collaborative, integrated team of professionals in support of our Adolescent HIV/AIDS Primary Care and Medical Case Management Program (Project IMPACT). MSAHC is a unique, New York City-based program that delivers comprehensive, integrated medical and mental health services and prevention education to young people aged 10 to 26. 

REQUIREMENTS:

·       Coordinate/actively participate in Project IMPACT community outreach by developing and maintaining community linkages and partnerships.

·       Maintain Project IMPACT outreach database.

·       Accurately enter/update/maintain patient care information into AIRS data system and facilitate data mining as needed.

·       Assist with facilitating weekly HIV+ support groups and related programming.

·       Prepare data as needed for the NY State AIDS Institute/AQLN. 

·       Patriciate in AQLN meetings and activities every quarter.

·       Implement quality improvement projects

·       Coordinate YAB Quarterly Meetings

·       Provide administrative support to program staff.

·       Provide administrative support to the Transgender Health Project.

·       Provide Patient Navigation to youth referred to Project Impact.

·       Assist current MSAHC Health Ed staff with HIV testing in the clinic.

·       Assist current Project Impact Staff with HIV PEP/ PrEP navigation.

·       Reports to the Project Impact Lead Physician

·       Expected to be based on-site at the Mount Sinai Adolescent Health Center.

Education Requirements:

Associate’s degree, Bachelor’s degree preferred, with a concentration in public health or community health a plus.

Experience Requirement:

Entry experience but 1+ years preferred

Licensing and Certification Requirements (if applicable)                                                      

Certification as a Certified Health Educator (CHES) from the National Commission for Health Education Credentialing, Inc. preferred
