    Community Living Corporation (CLC), an affiliate of CLC Group, is a nonprofit organization that provides residential and day hab services to developmentally disabled and dually diagnosed adults in North Westchester County.     Community Living Corporation is seeking a full-time or part-time Registered Nurse to join our organization. Duties include but not limited to:

    • Responsible for the day-to-day health care needs of all individuals in residential settings. Weekly visits onsite to each residence located in the Northern Westchester area are required as needed.
    • Responsible for reviewing doctor's orders prescribed by a Licensed Health Care Professional authorized to order such care daily.
    • Assess the individual's need, planning nursing care and review any changes or additions to nursing care plans to determine the appropriateness of delegated nursing tasks daily.
    • Review recent medical visits and communicates the health care needs of the individual to the appropriate CLC personnel and the individual's family as needed.
    • Administers medication on an occasional or emergency basis.
    • Provides supervision of staff performing delegated nursing tasks when necessary
    • Oversees for the proper storage, accounting and disposal of all controlled and non-controlled prescription medications as needed.
    • Provides nursing and health counseling and education to the clients and personnel.
    • Insures consistent follow up to all medical treatment plans and records.
    • In conjunction with the Director of the Department, identifies and presents any infectious disease risk and works in minimizing such risk in the workplace. Implements policies and procedures to ensure risks do not affect the health, safety and welfare of the clients or personnel daily.
    • Serves as an active liaison between CLC and the medical community. Includes computer communication with pharmacies, medical vendors, families and other services.
    • Monitor and follow-up with clients, physicians, residential staff, clients’ families/advocates and Service Coordinators on medical issues when needed.
    • Attend case conferences and ISP’s when necessary.
    • Participates in discharge coordination with hospitals, rehabilitation facilities etc.
    • When needed or recommended, attend rehab care plan meetings, priority medical appointments, surrogate committee meetings, hospital visits et al.
    • Monitor all medication changes and adjustments.
    • Responsible for supervision of AMAP recertification testing and recertification pouring’s
    • Monitor and supervise AMAP staff on site at residence. Includes pouring’s for initial AMAP certification. Ensure AMAP staff complies with OPWDD regulations and physicians’ medication orders. Ensure follow-up appointments when necessary.
    • Reinforces the mission of CLC by providing the finest possible level of professional care daily.

    Prior experience working with developmentally disabled individuals highly preferred. Ability to interact and communicate well with individuals with developmental and psychiatric disabilities.

    Must be licensed and currently registered to practice in New York State. Must have valid driver's license for a minimum of 3 years, and own transportation. Background check and drug testing required.
