About Us

The Health & Housing Consortium (“the Consortium”) is a collaborative network of health care, housing, homeless and social services organizations, and government partners with the shared goal of improving health equity and housing stability by fostering cross-sector relationships, informing policy, and building the capacity of frontline workers to support people with unmet health and housing needs. 

The Bronx Health & Housing Consortium started in the Bronx in 2011 as a response to healthcare reform, specifically the redesign of New York State’s Medicaid program. Understanding the critical role that housing has on health, the Consortium formed to help break down the barriers that existed between the health care, housing, and homeless services sectors, improve institutional communication and coordination of care, and provide support to frontline workers serving people with unmet health and housing needs. The Consortium is a groundbreaking organization that brings together hospitals, managed care organizations, housing and homeless services providers, and other community-based organizations and advocates, together with City and State government agencies to collectively work on issues at the intersection of health and housing in NYC.

In 2017, the Consortium incorporated as The Health & Housing Consortium, Inc. and beginning in 2018, supported the replication of our model in Brooklyn through a partnership with NYU Langone Health’s Department of Population Health. Learn more at our website: www.healthandhousingconsortium.org


The Consortium achieves its mission through three major areas of focus:

(1) Research & Advocacy, to better understand the scope of people with health and housing issues, their unique needs, the gaps in the systems that they touch, and how can we collectively address those gaps;

(2) Training & Resource Development, to help the people who are serving this population to get the information and resources they need in order to better support their clients/patients; and

(3) Cross-Sector Collaboration, to provide forums for people from different sectors and organizations to meet, share knowledge, identify common issues, and build relationships.
