    Description

    Role Overview

    We are seeking a licensed clinician to volunteer as a Community Health Worker. The volunteer clinician will conduct clinical assessments to help determine housing eligibility and connect participants to appropriate services.

    Key Responsibilities

    Conduct comprehensive clinical and psychosocial assessments

    Evaluate mental health, substance use, and medical needs

    Provide documentation to support housing placement

    Coordinate with case managers, housing providers, and community partners

    Uphold confidentiality and follow all compliance standards

    Qualifications

    Current clinical license (e.g., LCSW, LMFT, LPCC, RN, NP, or similar)

    Experience with homeless and/or justice-involved populations preferred

    Commitment to trauma-informed, culturally responsive care

    Strong communication and documentation skills

    Flexible schedule; even a few hours weekly can make a difference

    Remote and/or community-based assessments depending on participant needs
