Below are the position descriptions for positions at the University of Chicago Medical Center and Ingalls Memorial Hospital.
AmeriCorps is a national service program that involves people in “getting things done” in communities. AmeriCorps members develop an ethic of service while strengthening local communities. Each year in Illinois, thousands of AmeriCorps members give millions of dollars in service back to their communities. For up-to-date information on the impact on our state, visit AmeriCorps.gov.
Serve Illinois administers 45 AmeriCorps State programs in operation for the 2021-22 program year and over 1,800 Members committing to a year of service. These Members serve 70+ counties in over 400 host sites.
The Comprehensive Care, Community and Culture Program (C4P) aims to improve the overall health and well-being of patients in the program by addressing a wide range of medical and social needs. The C4P program builds off of the Comprehensive Care Physician Program, which offers patients at high risk of hospitalization a comprehensive care physician (CCP) who cares for them in both the hospital and the clinic. In addition to receiving care from a CCP, C4P patients are regularly asked about a range of unmet social needs and given access to a community health worker and community-based arts and culture programming. The Robert Wood Johnson Foundation has funded a randomized trial of C4P at the University of Chicago.
The CCP and C4P Programs are currently being expanded to and studied within new health care settings, including community hospitals like Ingalls Memorial Hospital.
The Comprehensive Care, Community and Culture Program (C4P) is proud to be an Illinois State AmeriCorps program. C4P will host up to 17 full time members across all of its participating service sites in 2021-22.
We are currently accepting applications for two sites:
Applicants may apply to both sites but they must submit a separate application to both application portals (see below for more information).
Members based at Ingalls Memorial Hospital will spend 1-2 months at the University of Chicago for training at the beginning of their service and typically come to UChicago for service activities 1-2 days per week during the service term.
AmeriCorps members will be responsible for extending the reach of the C4P program to impact patient experience with care and health outcomes, lower hospitalization rates, and resolve unmet social needs for participants.
