About the Role:

We are seeking an experienced and compassionate Home Care Field Nurse to join our team, working with adult and geriatric patients. The ideal candidate will have a strong understanding of Managed Long-Term Care (MLTC) and community health nursing, with a focus on creating patient-centered care plans and achieving positive clinical outcomes.

Key Responsibilities:

• Develop and maintain comprehensive, interdisciplinary home health care plans in collaboration with patients, families, and physicians.
• Regularly update care plans to reflect changes in patient needs and goals.
• Case manage care and services to achieve positive clinical outcomes and minimize rehospitalization risks.
• Support agency financial goals through effective resource utilization and cost management.
• Demonstrate expertise in community health nursing, providing care that aligns with best practices and regulatory standards.

Qualifications:

• Education: Bachelor of Science in Nursing (BSN) preferred.
• Licensure: Current and unrestricted Registered Nurse (RN) license in NY.
• Experience: Familiarity with MLTC and community health nursing is required. Experience in home care and geriatric nursing is a plus.
• Excellent clinical assessment, case management, and communication skills.
• Proficiency in interdisciplinary care planning and collaboration.
