Pathways to Housing DC (Pathways DC) is an innovative nonprofit agency committed to ending homelessness for individuals with serious mental illnesses, co-occurring substance use disorders and/or other medical challenges. Pathways is one of the originators of "Housing First", in which services are provided to those in need without conditions and pre-housing requirements such as curfews, mandated sobriety or compliance with medication.

Case Managers are members of the Montgomery County, MD Housing Initiative Program (HIP) who provide flexible, community based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals. Case Managers maintain a primary caseload of up to 18 clients and provides services in a compassionate and responsible manner.

RESPONSIBILITIES:

  • Collaborates with client in the formulation of a Case Management Plan that is reviewed and modified with client on a regular basis to provide individually tailored services.
  • Provides outreach, case management, counseling, referrals, advocacy, and other needed services to clients in their home.
  • Completes MA, SSI/SSDI, and SNAP applications with clients; compile reports and complete program documentation.
  • Maintains written and computerized records, compiles reports and completes program documentation such as: progress notes, case plans, intakes, assessments, monthly visit reports, incident reports.
  • Meets HIP contractual billing and documentation guidelines which include: monthly client contact requirements which are dictated by the assessed level of need from 1-4 home visits a month; case notes entered within 24 hours of visit, maintain housing stability, and increase client income.

  • Participates in weekly staff meetings;
  • Participates in staff trainings as scheduled;
  • Other duties as assigned

QUALIFICATIONS:

  • Bachelor’s Degree in related social service or healthcare related discipline
  • Two years related experience providing human services and/or training and working knowledge of social service and individual support services, Medicaid services, State and Montgomery County community resources
  • Excellent organizational and both written and oral communication skills with the ability to communicate effectively with individuals with varied cognitive abilities to establish professional relationships
  • Ability to work independently
  • Excellent computer skills
  • Patience, creativity, flexibility, compassion, and sensitivity to persons with
    disabilities and other minority populations;
  • English/Spanish strongly desired + $5BONUS OFFERED;
  • Driver’s License and access to personal vehicle required;
