The Department of Human Service Programs is seeking an experienced professional to oversee the implementation and operations of Cambridge’s Coordinated Entry system, Cambridge Coordinated Access Network (CCAN). The Coordinated Entry system aims to equitably, efficiently and effectively connect people at risk of or experiencing homelessness to resources to address their housing crisis. Reporting to the Director of the Multi-Service Center, this position will supervise two CCAN Specialist positions and will coordinate multiple external partner agencies participating in the Coordinated Entry system.

  • Provides leadership, direction and supervision to staff responsible for implementation of the coordinated entry system.
  • Serves as the point of contact with the Continuum of Care (CoC) and Homeless Management Information System (HMIS) staff regarding the Coordinated Entry system and assists in establishing and implementing policies and procedures to ensure households at risk of or experiencing homelessness are able to navigate the system and end their housing crisis.
  • Ensures compliance with Coordinated Entry requirements defined by the U.S. Department of Housing and Urban Development (HUD) and oversees key functions of the system including access, assessment, prioritization and referral procedures.
  • Manages activities related to CE project management, including but not limited to:
  • Facilitation of case conferencing
  • Vacancy and referral oversight and tracking
  • Coordination of housing navigation teams
  • Phone or email correspondence with clients for 90-day check-ins
  • Ensures coverage of access points
  • Assistance establishing training protocols for consistency of triage, intake and assessment tools
  • Maintains, updates, and oversees implementation of policies and procedures using stakeholder input to ensure compliance with HUD requirements and adherence to system performance goals.
  • Trains and monitors relevant staff and partner agencies on use of assessment tools and referral procedures.
  • Collects data and monitors data quality in HMIS for the Coordinated Entry System for the purposes of system performance monitoring, project performance monitoring, continuous quality improvement, and meeting HUD reporting requirements.
  • Builds and maintains strong, collaborative working relationships with internal and external partner agencies and stakeholders to improve coordination of referrals to and from the CE system.
  • Supports the work of the Continuum of Care (CoC) Coordinated Entry Working Group and other leadership bodies to ensure stakeholder input is incorporated into policy and implementation decisions; prepare periodic reports and updates to track progress of strategic goals and system performance.
  • Reviews data to analyze outcomes pertaining to referrals, denials and housing placements.
  • Evaluates system performance specific to racial disparities and ensuring equity, revises and/or implements new processes and refinements as needed.
  • Actively engages in the Department’s Race and Equity Initiative.
  • Develops marketing material for people seeking assistance and general public that will increase awareness of access points.
  • Represents the Coordinated Entry system at meetings with various organizations, coalitions and task forces as assigned.

MINIMUM REQUIREMENTS:

  • Minimum of four years of experience in human service program management or service delivery, program evaluation, stakeholder management or related professional experience is required, including two years in a project or program management capacity. Additional supervisory experience preferred. A bachelor’s degree in Policy, Public Administration, Public Health, Business or Social Work may be substituted for two years of the required experience.
  • Strong familiarity with Cambridge community, service systems and the various types of local, state and federal funding sources for housing and homeless services.
  • Strong communication, organizational and interpersonal skills.
  • Demonstrated ability to successfully implement and operate projects that rely on multiple internal and external stakeholders from nonprofit and public sectors.
  • Demonstrated ability to produce effective policies and procedures, training materials, and presentations.
  • Proficiency in personal computer skills such as Microsoft Office Suite; experience with HMIS or other database system/s.
  • Ability to read and interpret HUD and other pertinent federal regulations and effectively disseminate related information; and
  • Ability to adhere and commit to established workplace behavioral guidelines.

WORK ENVIRONMENT

Most work takes place in an office setting with fluorescent lighting, carpeted floors, and air conditioning. Required to attend meetings in various agencies and settings in Cambridge and other nearby localities, so must have the ability to travel to various sites.

 PHYSICAL DEMANDS

Ability to use typical office equipment in an office setting; ability to travel to other local offices and surrounding agencies. 

RATE: $65,419-$79,607 plus excellent benefits
