Case Manager & Outreach Specialist

The Case Manager and Outreach Specialist provides client-centered individual support and coordination of care to clients in our harm reduction program for a diverse client population who are actively using or have past experience(s) using crystal methamphetamine use. The Case Manager and Outreach Specialist will support clients with benefits enrollment and help clients reduce barriers to care and increases accessible, available, and appropriate linkages to care for clients who use crystal methamphetamine. The Case Manager and Outreach Specialist will also be the primary lead on conducting outreach to potential clients in various venues and settings, which includes many traditional and non-traditional methods of outreach.

 

Essential Job Functions 

The following duties are mandatory requirements of the job:

·      Provide coordination and navigation support to clients in accessing and enrolling in various social programs, public benefits, and healthcare. These include, and are not limited to, insurance and SNAP benefits enrollment, housing program applications, and healthcare and substance use programs enrollment.

·      Provide care coordination and linkage with clients’ other care managers, patient navigators, medical doctors, psychiatrists, counselors, and other social/healthcare providers.

·      Assist in the provision of education, risk reduction, treatment adherence, and referral services to clients regarding HIV/AIDS, STIs, PrEP, and PEP services.

·      Conduct outreach to promote, link, and engage clients into our crystal meth harm reduction program.

·      This includes both traditional and non-traditional outreach such as street outreach, collaborating with community organizations and partners, visits to medical facilities; outreaching at circuit parties, clubs, bars, and bathhouses; and outreaching in other appropriate spaces for engagement of potential clients.

·      Support with digital promotion and outreach on social media and other platforms, such as dating apps, Facebook, Twitter, Instagram, etc.

 

Other Responsibilities

The following duties may also be performed as assigned by the supervisor:

·      Provide screenings and assessments of potential clients and assess for appropriate level of care, with specific focus on crystal meth use and sexual behaviors.

·      Collaborate with a team of peer workers to conduct outreach events.

·      Attend program and agency meetings and/or other trainings.

·      Provide all required documentation/statistics accurately and in a timely manner.

·      Assist with special projects and perform other related duties as requested.

 

Education and Certification 

·      Bachelor’s degree in behavioral health, or social science

·      CASAC certification by OASAS in the state of New York a plus

 

Special Skills and Knowledge 

In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:

·      Proven experience working with populations that use crystal meth is required.

·      Knowledge of HIV/AIDS and experience working with LGBTQ-identified adults is required.

·      Excellent communication, organizational, and interpersonal skills along with skilled documentation and computer competency are essential.

·      Possessing a nonjudgmental attitude when engaging clients who use substances.

·      Knowledge of screening and assessing clients with multiple needs.

·      Demonstrated ability to work effectively as part of an interdisciplinary treatment team as well as independently are essential.

·      Must be able to develop and maintain interagency liaisons as a means of meeting clients' needs.

·      Must be flexible with late night work hours.

·      Bilingual in English and Spanish preferred.
