The Stanley M. Isaacs Neighborhood Center (Isaacs Center) is a non-profit, multi-service organization focusing primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.  


The Cluster Director of East Harlem Aging Services will oversee day to day operations of a case management based aging services program for older adults residing in New York City Housing Authority buildings in the East Harlem neighborhood of New York City and surrounding community, and will report directly to the Associate Deputy Executive Director. This role will also include a lead role in the execution of new contracts designated for services in East Harlem. Provision of services will be in accordance with Isaacs Center and Goddard Riverside policies, contractual guidelines and standards.

The Cluster Director must possess excellent writing, speaking, interpersonal and organizational skills. Must hold at least a Bachelor's Degree, LMSW or LCSW is preferred. The Cluster Director must have demonstrated experience providing exemplary supervision, direct clinical practice, group facilitation, and managing data and outcomes. The candidate must have a history of working with diverse and high-needs aging populations, value team building, and support the development of community to address complex issues. Direct experience of practice in East Harlem is highly preferred.


Goddard Riverside and Isaacs Center follow the CDC and NYS recommendations to prevent the spread of COVID-19, and are now requiring all new hires to be vaccinated against COVID-19 unless they have a qualified exemption. 


Education/Skills/Experience:

• Bachelor’s Degree, Master’s Degree, LMSW or LCSW required

• A SIFI field instruction certification required

• Prior experience working with the elderly highly preferred

• Bilingual, English/Spanish highly preferred

• Ability to handle confidential information with discretion

• Ability to work some flexible hours (e.g., evenings, weekends, Thanksgiving or Christmas Holiday) 


Program Oversight and Administration

• Supervise the home-based and in-person services to seniors residing within the housing development and surrounding areas

• Provide direct supervision to Clinical Manager, and identified program staff and student interns

• Oversee and coordinate education and recreation programs and healthcare services through staff, volunteers, consultants and community partner

• Oversee the develop of case plans to promote client self-determination; to assist seniors with staying in the community; and to promote healthy and independent aging.  

• Assist in the implementation of new contracts designated for East Harlem and surrounding areas

Operational and Contractual Expectations

• Manage and track contract requirements for government contracts and foundation support

• Manage budget for hybrid model service delivery 

• Fundraising and grant writing alongside Development staff

• Complete case notes , monthly dashboard, and log outcome measurement data

• Prepare reports, and submit monthly reporting to ADED and fiscal operations

• Prepare documentation (assessments, case logs, case notes, etc.) as stipulated by contractual and agency standards and procedures.

• Oversee facilities and safety needs for the Older Adult Center site at 1365 5th Ave 

o Ensure that all staff and visitors to the facility follow established check-in process 

o Execute and enforce all health and safety protocol set forth by the Director of Operations and ADED

o Complete daily facility checks and report issues to NYCHA and the Isaacs Center facilities department as necessary 

• Partner with the Director of Operations on both short- and long-term capital improvement planning

Community Engagement and Partnership

• Assist and supervise outreach efforts to increase participation, including supervision of Community Engagement team

• Oversee ongoing community needs assessments and tailor/build programming in response to these findings

• Collaborate with community agencies to implement in-home services such as visiting nurse services, home care, food pantry, and other needs.  

• Develop and foster relationships with community members, local service providers, and government agencies.  

• Maintain presence with other sector leadership through citywide meetings, Community Board, and others
