    Job Title: Social Care Network (SCN) Performance Coordinator

    Reports to: Chief Clinical Officer/Director of Data Analytics

    Project Summary:

    The New York State Department of Health has established Social Care Networks (SCNs) as part of the 1115 Waiver Demonstration Amendment, and Forward Leading IPA (FLIPA) is the SCN Lead for the Finger Lakes Region. This region is comprised of 14 counties, including Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, and Yates. The SCN’s aim is to enhance the delivery of social care services to Medicaid members by coordinating efforts among community-based organizations (CBOs) and other health care partners to create a more resilient, flexible, and accessible social care system that reduces health disparities and advances health equity. The SCN will collaborate with CBOs and other health care partners, leveraging shared data and technology to coordinate social care services for Medicaid members to improve access, ensure reliable and timely referrals, streamline and track navigation and completed referrals in closed loop systems and enhance collaboration between social care service providers and other regional partners.

    Organizational Overview:

    Forward Leading IPA (FLIPA) is a nonprofit membership association of safety net providers working in partnership to provide the highest quality integrated healthcare to historically underserved populations in Upstate New York since 2017. FLIPA is renowned for its commitment to integrating primary care, behavioral health, and social care needs. Our growing membership serves individuals across more than 27 counties and includes federally qualified health centers (FQHCs), behavioral health providers, and a rural health network consisting of eight county public health departments.

    Job Summary: The SCN Performance Coordinator works with the SCN Project Director, SCN Partner Liaison Specialists, Compliance Officer, and partners to ensure the FLIPA SCN and engaged CBOs meet performance and compliance milestones through training, reporting, and coordination of necessary supports for performance improvement. This position is grant-funded through March 2027.

    Duties/ Responsibilities:

    • In collaboration with FLIPA SCN leadership, develop project plan that includes required performance milestones, timelines, data flows, and required key performance indicators
    • In collaboration with FLIPA SCN leadership, develop user-friendly, clear, and concise dashboards/reporting with appropriate performance metrics to assess and monitor SCN performance information on a daily/weekly/monthly basis as appropriate to metric
    • Ensure dashboards and metrics support actionable system and individual CBO performance improvement
    • Use dashboards and related data and information to identify performance improvement opportunities at both a systems and individual CBO level
    • Support Partner Liaison Specialists, vendors, and FLIPA SCN leadership in identifying opportunities and implementing strategies to close performance gaps
    • Coordinate needed technical assistance and training support
    • Organize and communicate findings and concerns to team members
    • Ensure project activities are assigned to appropriate resources for completion
    • Monitor project progress and proactively work to identify and mitigate risks and barriers that may impact implementation of project activities and goals
    • Create, organize and maintain comprehensive project performance documentation, remediation and training plans, and reports
    • Establish and manage resource library and learning management system specific to commonly identified performance improvement training needs
    • Effectively work across multiple organizations and geographies to support project progress within different organizational culture structures and communities
    • Other duties as assigned

    Education & Experience:

    • Master's preferred in related field, such as business, public health, healthcare administration or equivalent experience required
    • 3-5 years of change management, project management or related experience required
    • Prior experience managing performance within large-scale grants/projects preferred
    • Experience within the New York State health care system (population health, behavioral health, Federally Qualified Health Centers, or similar) required

    • Excellent organizational and leadership abilities
    • Ability to manage multiple priorities effectively with minimal supervision
    • Outstanding communication and people skills
    • Ability to gather information throughout the lifespan of a project and report out to a variety of audiences
    • Experience seeing project through the full life cycle
    • Strong interpersonal skills, independently resourceful and thrives in a fully remote environment
    • Proven ability to complete projects according to defined timelines and objectives
    • Proficient with MS Office and various business software

    Salary Range:

    • Salary is commensurate to education and experience with range of $75-95k
    • Additional information:
    • This position will be remote with regular travel expected for on-site meetings or training across the Finger Lakes Region and New York State
