The Brook Supportive Housing Program has multiple opportunities for Clinical Case Managers to join an established clinical team. The Brook permanently houses 120 special needs tenants, the majority are formerly street homeless and have serious mental illness, chronic illness (including HIV/AIDS) and substance abuse challenges. Clinical Case Managers provide psychosocial support, medication monitoring, financial management assistance and supportive case management services to single adults in a community environment.
Ideal candidate can demonstrate ability to work in partnership with clients from a strengths-based perspective, overcoming stigma, and navigating complex systems. Additionally, ideal candidate can adapt to an active environment, is exceptional with time management, has strong writing and communications skills, exhibits excellent attention to detail and remains a team player. The Clinical Case Manager is responsible for providing program and staff support to the Clinical Coordinators and working in collaboration with the team social workers and case managers. The Clinical Case Manager is deeply involved in providing direct services to 120 special needs and formerly homeless individuals, that have serious mental illness, chronic illness (including HIV/AIDS) and substance abuse challenges. The Clinical Case Manager will be responsible for providing direct clinical care to residents as well as facilitate groups and workshops with an emphasis on behavioral health needs. The Clinical Case Manager will contribute to the continued improvement and excellence of the program.
The Clinical Case Manager will be responsible for the oversight of clinical assessments conducted by Case Managers, Complete HRA required psycho-socials, manage the coverage needs of Clinical Coordinators, run team reports, assist with task f/u and provide administrative oversight of team caseloads.
Clinical Case Managers/ will be responsible for the delivery of direct services to tenants. Clinical Case Managers will be responsible for working with tenants to ensure that they understand their liabilities and meet all obligations listed in the lease agreement in order to maintain permanent housing. Clinical Case Managers will also provide, supervise and direct clinical services with tenants who have more intense psychiatric and behavioral health needs. Clinical Case Managers will coordinate services for our tenants that include medical care, psychiatric care, community living skills, financial management, information and referral, and substance abuse care. Clinical Case Managers will have a master’s degree in a mental health field such as Social Work and/or Mental Health Counseling.
• Provide direct services to clients
• Maintain case records and statistics for program with the goal of analyzing trends and improving service delivery
• Assist the program director in recruiting and training direct service staff
• Ensure all programs meet contractual goals and reporting requirements
• Submit internal progress reports on a monthly basis, or more frequently as required
• perform case reviews and structured observations
• Participate in, and encourage staff to become involved in agency events
• Act as liaison with the community, including the area community district and board
• Perform additional duties as assigned
• Master’s degree required
• Minimum of One-year program experience
• Proficiency in modern business communications including person computers, electronic mail, voicemail, facsimile and copier equipment
