Outreach Specialist 

 

Summary 

Swords to Plowshares will re-engage in outreach to Homeless Veterans on the streets of San Francisco starting on October 1st, 2021. As it was in the past, Outreach will run through the San Francisco Drop-in Center, supervised by the Grant per Diem Coordinator. It is anticipated that Swords will hire two new Outreach Specialists. Ideally, both Outreach Specialists will have been or will have had experience with homeless veterans. Outreach Specialists, will, to the extent possible, work as a team of two when going out on the street. They will provide a schedule for the week to their supervisor at the beginning of the week and will check-in with their supervisor to document progress in making contact with veterans.

Roles and Responsibilities 

Priority 1: Homeless Veterans on the by-Name List (BNL): The Outreach team (outreach) will find and engage with homeless veterans on the BNL on a regular (at least monthly) basis. Outreach will… 

  • Find homeless veterans on the BNL on the streets of SF 
  • If not enrolled in ECM, enroll in ECM 
  • Document location in the One System 
  • Engage in a problem-solving conversation with the vet and provide resources related to problem solving, if warranted or 
  • Refer to STP Problem-solving at 1060 Howard 
  • Engage in a Primary Assessment with the veteran, if needed 
  • Gather documents needed for navigation into permanent housing 
  • Refer to Navigation at 1060 Howard 

Priority 2: Homeless Veterans not on the By-Name List (BNL): Outreach will find and engage with homeless veterans who are not on the BNL on a regular (at least monthly) basis. Outreach will… 

  • Find undocumented and unenrolled homeless veterans on the streets of SF 
  • Enroll veteran in ECM and the One System 
  • Document location in the One System 
  • Engage in a problem-solving conversation with the vet and provide resources related to problem solving, if warranted or 
  • Refer to STP Problem-solving at 1060 Howard 
  • Engage in a Primary Assessment with the veteran, if needed 
  • Gather documents needed for navigation into permanent housing 
  • Refer to Navigation at 1060 Howard 

Priority 3: Sweeps of homeless veteran hotspots to find new homeless veterans: Outreach will develop several regular “routes” that will “sweep” through hotspots like Bart Stations, Glide, the TL, 6th Street and other known regular locations that are known to be regular hang-outs of homeless veterans. Outreach will ask if there are any homeless veterans and will… 

  • Find homeless veterans on the streets of SF
  • Enroll veteran in ECM and the One System
  • Document location in the One System
  • Engage in a problem-solving conversation with the vet and provide resources related to problem solving, if warranted or

Priority 4: Transportation for Veterans associated with Drop-in, the Access Point or SSVF: When possible and when not interfering with any of the previous three priorities, Outreach Specialist will drive the minivans to facilitate transportation for the Drop-in Center, the Access Point and SSVF. This can include… 

  • Transporting homeless veterans to and from 1060 Howard. 
  • Transporting homeless veterans to and from the VA Downtown Clinic and Fort Miley (SFVAMC). 
  • Transporting homeless veterans to and from various agencies to gather paperwork related to navigating toward permanent housing 
  • Transporting homeless veterans to and from permanent housing locations to view the locations and sign leases. 
  • Assisting with move-in to permanent housing for homeless veterans 

 

Requirements 

Education 

  • Two-year college degree or equivalent combination of education and/or work experience 

Experience 

  • Translatable military experience and skills may be alternatively substituted 
  • Lived experience as a veteran or homeless person or both
  • Experience conducting outreach to homeless people

Qualifications 

  • Strong initiative and enthusiasm and willingness to pitch in whenever needed 
  • Employee must be able to perform all essential functions as described within what would be considered reasonable accommodations. 
  • Clean DMV record 
  • Treat all clients and colleagues with dignity and respect 
  • Must be able to communicate effectively with a variety of personalities and be comfortable working with an at-risk client population, including embracing the agency’s DEI (Diversity, Equity and Inclusion) initiative 

Skills 

  • Ability to work in a team environment and meet regularly with Drop-In Program Staff to provide?outreach?updates.  
  • Excellent customer service skills  
  • Excellent organizational and time management skills  
  • Flexibility and willingness to perform multiple jobs in a work environment with changing priorities  
  • Strong problem-solving skills  
  • Ability to follow through on assigned tasks 

 

Working Conditions 

Position requires prolonged walking, driving, sitting, and repetitive hand movement using a computer keyboard in the performance of daily office activities.
