PATH is seeking master’s level social services candidates to join our San Jose team as a Clinical Case Manager. This position will join our San Jose street outreach team as we expand our services in the Downtown San Jose area.

ABOUT OUR STREET OUTREACH PROGRAMS
Our Street Outreach programs work to engage individuals who are currently homeless and living on the streets in an effort to provide and link them with services that will ultimately lead them to stabilize in housing long-term.
WHAT IS A CLINICAL CASE MANAGER?
As part of the Outreach team, the Clinical Case Manager helps clients break the cycle of homelessness by providing individualized and comprehensive support throughout their transition from street to housing. Responsibilities of the Clinical Case Manager include:
