Organizational Description:

Sunnyside Community Services (SCS) is a multi-service, community-based organization providing a broad range of services to approximately 16,000 people of each year. Our services include pre-kindergarten and after school programs; college prep and career programs for teens and young adults; supports for immigrants; a vibrant center for older adults, dedicated home care services, support and assistance for family caregivers, and social adult day program for people with cognitive and physical impairments.

Role Description:

SCS is seeking an Intake Specialist who will be a part of the Senior Services – Care Connections team. As the Intake Specialist, you will assess clients and make seamless connections to our continuum of programs and services for older adults by completing an universal intake. The universal intake gathers information specific to the individual and connects them to any one of our services for older adults and/or caregivers that can meet their needs. [SN1] Connections to services include access to counseling both in the home and in the office, access to meals delivered to their home, home care, Medicaid enrollment, membership to our older adult center and services to families caring for an older adult, someone living with memory loss or grandparents caring for grandkids and more.

The Intake Specialist will also have access to agency resources to properly refer a prospective client to the appropriate services within SCS’s immigrant, youth, and home care services or to external sources to best meet their needs. They will report to the Director of Caregiving – Alzheimer’s Services with day-to-day direction of the Care Connections Manager.

Responsibilities:

·        Provide information, education and customer service to prospective clients.

·        Good record keeping, computer knowledge and time management skills are essential.

·        Conduct telephone and in person intakes.

·        Screen and assess individuals for program eligibility and ensure compliance with contract requirements where applicable.

·        Identify and assist clients with basic information on benefits and entitlements.

·        Maintain complete and accurate data in Salesforce and Peer Place as required by DFTA and SCS.

·        Make referrals to other available resources.

·        Participate in trainings, conferences, and agency meetings

Physical Requirements:

This role will require bending, sitting, stooping, and standing.

Working Conditions:

The work of this role will take place in an office setting.

Job Qualifications:

·        A minimum of a bachelor's degree in a social services field.

·        Experience working in social services required. Experience working with families and geriatric populations preferred.

·        Ability to communicate effectively with other team members, community agencies, vendors, and clients with diverse opinions, values, and cultures.

·        Excellent interpersonal communication skills. Ability to develop and maintain effective working relationships with caregivers, staff, volunteers, vendors, and other community/government agencies.

·        Ability to provide accurate and timely documentation required.

·        Strong computer skills with experience in Microsoft Word, Outlook, and Excel. Salesforce experience is preferred.

·        Ability to work independently and in a team environment.

·        Must be bilingual – Spanish required.
