Health Care Navigator

 HCS is looking for a Health Care Navigator to join our Supportive Services to Veteran Families (SSVF) Program staff. We are looking for a high energy, committed and quick learner to work with our SSVF team to support the needs of low-income Veteran households seeking stable, permanent housing.     

Conduct comprehensive (non-clinical) assessments of Veterans’ situations, service needs, and barriers to accessing care;

·      Assist Veterans define and communicate their preferences in care and health-related goals;

·      Assist Veterans identify and engage appropriate resources to address their medical needs;

·       Coordinate referral of Veterans to VA medical centers, community health clinics, and other programs needed to ensure Veterans’ appropriate access to health care;

·       Participate in the development of Veterans’ care plans, regularly review and evaluate Veterans’ progress with care plans, and provide appropriate resources and referrals to support care plan adherence;

·       Collaborate with VA and other service providers in the ongoing reassessment of Veterans’ health care needs;

·       Support Veterans towards optimizing treatment interventions and outcomes;

·       Provide ongoing health education support to Veterans and their families;

·       Assist Veterans maintain open communication with their care provider and/or treatment team;

·       Connect Veterans to appropriate support services including housing, financial benefits, and transportation;

·       Act as a liaison and develop new partnerships and relationships with VA staff, community health care providers and other appropriate health networks.

POSITION QUALIFICATIONS:

·       Master of Social Work or comparable advanced degree;

·       Candidates with professional/practicum experience in a hospital, community- based health organization, VA Medical Center, or other medical related setting strongly encouraged to apply;

·       Strong ability to work independently and as a team player exercising mature judgment;

·       Highly motivated self-starter with an ability to complete multiple tasks in timely manner;

·       Ability to develop partnerships with community organizations and government agencies;

·       Must have excellent problem-solving skills and able to develop innovate solutions;

·       Experience providing supportive services to at-risk populations;

·       Ability to respond to multiple emergency needs with patience and diligence;

·       Must demonstrate the ability to communicate with a variety of professionals including doctors, social workers, attorneys, contractors, and landlords;

·       Must maintain and execute confidential information;

·       Strong oral and written communication skills as well as excellent computer skills;

·       Ability to translate technical information into layman’s terms;

·       Sensitivity to the needs within the community and to individual clients;
