    Values Into Action, a leader in person-centered and community-focused services in partnership with people with disabilities, is looking for a determined and energetic Community Support Facilitator to work throughout Philadelphia and Delaware County. We are looking for someone with strong leadership skills and who understands the distinct nature of working on a residential services team.

    Values into Action supports adults with intellectual and developmental disabilities one person at a time, on their own terms, and exclusively in their own homes and communities. As a Community Support Facilitator, you will learn what is important to the person in their life and support them in achieving it. You will also lead the development, ongoing supervision, and coordination of the person’s team of Support Partners with a focus on community involvement and/or accommodation with all activities of daily living including personal care, communication, transportation, and inclusive social and recreational activities.

    Some responsibilities will include the following:

    • Provide information, guidance and technical assistance to the people who choose to utilize our services so that they can access, organize and utilize opportunities, resources and services necessary to live a self-determined life.
    • Provide leadership, supervision, coaching and mentoring to Support Partners.
    • Coordinate schedules that meet the staffing needs and preferences of persons with ID/DD, informing recruitment efforts and facilitating staffing interviews to cultivate strong Support Partner teams.
    • Play a key role in the person's team, helping to plan services, and mentoring the team to ensure the happiness, health and safety of the person supported;
    • Provide direct services to individuals with ID/DD that encourages community participation and inclusion to help persons accepting support to make meaningful contributions in their community.

    Qualifications:

    • 18 years of age or older
    • to be considered for the Community Support Facilitator role, candidates must reside in Philadelphia or Delaware County, as this position requires flexibility with working day, evening, overnight and/or weekend hours, participation in an after-hours/weekend on-call rotation, and travel via personal vehicle to areas within Philadelphia and Delaware Counties and surrounding counties.
    • Preferred is a bachelor’s degree from an accredited college or university and 2 years work experience directly with persons with intellectual and/or developmental disabilities; or an associate’s degree or 60 credit hours from an accredited college or university and 4 years work experience working directly with persons with developmental and/or intellectual disabilities. Required is a high school diploma from an accredited high or secondary school and 6 years’ experience working directly with persons with developmental and/or intellectual disabilities
    • Criminal history free from conviction/s that could pose a health and safety risk for people supported.
    • Be or willing to be Covid-19 Vaccinated prior to employment.
    • Reliable transportation to and from work and during the course of work, which may include a valid driver’s license, vehicle and a safe driving record, and/or knowledge of the public transportation system, as necessitated by the persons support.
    • Demonstrated commitment to the principles of person centered thinking, planning and action.
    • Tech savvy with proficiency in Microsoft Office
    • Ability to communicate effectively through written, oral and electronic means i.e. email, text, web
    • Ability to move about and to kneel, bend, squat, and reach as components of personal support.
