Counseling Resource Clinician - LCSW

Responsible for providing support to clinical SW/ Spiritual Care management of cases.

Responsible for evaluating department member’s clinical performance on an annual basis.

Responsibilities:

1. Assists in resolving complex clinical situations.
2. Acts as a resource for clinical staff
3. Attend weekly team meetings
4. Evaluates department members clinical performance annually in a field observation. (SW only)
5. Participate in Performance Improvement activities as appropriate
6. Ensures compliance with policies and procedures, CHAP, NYS standards and Medicare Conditions of Participation.
7. Any other duties as assigned.

Qualifications:

1. Master’s Degree in SW.
2. Licensed by State of New York as an LCSW.
3. Knowledge of End-of-Life theory/practice
4. Five years supervisory experience.
5. Experience with Inter disciplinary Model of Care
