The Trans Housing Coalition (THC) seeks a full-time Case Manager to join a new and growing team. The Case Manager will be responsible for assisting program participants with housing, making home visits, linking to support and treatment services, and managing a caseload of up to 15 people.

THC works to uplift trans and gender non-conforming people of color impacted by chronic homelessness in Atlanta. To accomplish our mission, THC utilizes a Housing First approach to provide brief accommodation in emergencies and immediately work on securing permanent housing and providing intensive wraparound services. Housing First is an evidence-based program founded on the belief that housing is a basic human right. THC echoes this belief and seeks to offer barrier-free, harm reduction housing and culturally competent and respectful services to trans and gender non-conforming people impacted by substance abuse, un- or underemployment, lack of access to medical care, mental illness, and related issues or conditions.

DUTIES AND RESPONSIBILITIES

The Case Manager reports to THC Program Director and is expected to

Case Management

  • Maintain a caseload of up to 15 active clients.
  • Conduct ongoing outreach and engagement with homeless individuals.
  • Conduct screening referrals, intake, and assessment.
  • Develop person-centered service plans; assist and support program participants in establishing, pursuing, and achieving goals – including but not limited to therapy, substance abuse counseling, and gender confirmation services.
  • Meet with program participants individually through regular home visits or visits in other community settings to provide services, review progress toward goals, and reassess and revise service plans.
  • Provide or facilitate transportation for program-relevant appointments/meetings or other support services. 
  • Accompany clients using personal or public transportation as needed.
  • Coordinate care and services, including developing and sustaining effective relationships with community resources and service providers.
  • Assist with the public assistance application process, obtaining government identification documents, and income verification.
  • Coordinate with community clinical services to complete physical and mental health evaluations.
  • Monitor for warning signs of crisis and respond accordingly.
  • Maintains records in accordance with organizational standards. Enters data in a clear, accurate, and timely manner and promptly submits documentation when needed.

Housing Support

  • Assist with the housing voucher and other housing entitlement program applications.
  • Engage and create relationships with landlords and property managers in the community to ensure participants can succeed in their housing placements and feel safe, comfortable, and respected in their housing.
  • Ensure that the client’s transitional and permanent housing options are up to HUD-housing standards by conducting walkthroughs, following up with emergency work-orders as needed for clients, and advocating on behalf of the clients when housing or neighborhood conditions jeopardize their wellbeing.
  • Provide intensive and ongoing support to clients in crisis who require moves between emergency or other housing placements, when they are evicted, and/or when they are forced to leave their current housing.

Individual Support

  • Hold regularly scheduled meetings with clients as part of developing ongoing, supportive relationships that help clients realize their goals and live the lives they want to lead.
  • Provide support in all areas of seeking, getting and keeping employment and or education.
  • Facilitate linkage to life skills or mutual support groups and community-based social, cultural, or recreational opportunities.
  • Provide assertive engagement through routine client visits, including after-care once program participants are placed in housing and navigating challenges to help maintain housing.
  • Work in partnership with program participants to manage daily life tasks, such as shopping, meals, transportation, laundry, and scheduling other appointments when needed.

QUALIFICATIONS & EXPERIENCE

The ideal candidate will have expertise in providing outreach and support services engagement to individuals experiencing homelessness, developing and sustaining collateral relationships with other service providers, and working as a member of a support services team. The successful candidate should be committed to uplifting trans communities, flexible, organized, and detail-oriented. This is a unique opportunity to be a collaborator in establishing an innovative program directly supporting trans communities.

The ideal candidate will have:

  • Knowledge of, or experience with programs and services available to transgender and gender non-conforming people is required.
  • Knowledge of, or experience with homeless services, mental health services, addiction treatment services, and/or navigating the criminal injustice system.
  • Knowledge of, experience with, or a willingness to learn about and practice Housing First, Harm Reduction, Trauma-Informed Care, and Motivational interviewing.
  • Experience in providing non-clinical individual case management.
  • Flexible approach to working with clients and the ability to multitask and work well under pressure.
  • Ability to work independently and collaboratively as a member of a closely-knit team.
  • Strong verbal and written communication skills.
  • Proficiency in Microsoft Office, GSuite, or similar software.
  • Proficiency in Apricot or similar client charting database.
  • Must have a valid Georgia driver's license; personal transportation with liability insurance preferred.
  • High school diploma or high school equivalency required; associate or bachelor’s degree in social work, sociology, or related field strongly preferred.

COMPENSATION & BENEFITS

Work will occur between 8 am - 5 pm but will also include some evening, early morning, and possible on-call evening hours. THC is a new organization based in Atlanta, GA with no physical office space; as such, this is a remote position. However, candidates must be currently located in or willing to relocate to Atlanta, GA. The Case Manager will ultimately be responsible for creating and maintaining their own schedule with supervision from the THC Program Director.

Salary is commensurate with experience within the range of $48,000 - $56,000. Benefits are 100% employer-paid for employees and 75% for dependents and include medical, dental, vision, and basic life insurance. Additional benefits include reimbursement for business-related cell phone service, transportation, vehicle insurance, and modest stipends for professional development.
